Nausea Presyncope

Symptomtext

Patient experienced a near syncopal episode with associated nausea. The nurse practitioner was notified and intervened. The patient was laid flat with her feet elevated until the episode passed. No LOC. She was monitored for 10 minutes with periodic blood pressure checks until stable discharge home.

Symptomtext

Vasovagal response. Skin color became pale, white lips, but was still able to speak with family and nurses. Pt was shaking, diaphoretic, and rap pulse on assessment. Pt stated, "my ears started ringing, bad pain in my left arm, things got dark, and remembering sitting on the floor when I came to." 911 was called. After deep breathing exercises and cool compresses pt slowed breathing, pinks became pink, and blood pressure/HR came back in normal range for his age. O2 sats were 98%.

Symptomtext

Client called today to say she had numbness and felt cold to the area where she received the vaccine , the 2nd night after receiving the shot but massaged the area and it went away. Says it didn't last long but thought the vaccine may have caused that and was asking me if was and whether she should no longer receive any vaccines. I asked if she continued with numbness or cold feeling and she stated no. Explained to the VIS statement, asked if she still had hers and she replied no. I told her I would file a VAERS report, also to seek Medical doctor if she had any further questions.

Symptomtext

Patient given pfizer covid vaccine formulation for 12 and up when they should have been given the 5 to 11 formulation. Patient is aware of the error. Weight in within adult weight range. Parent has been advised to call if patient has any symptoms. As of 5/26/22, parent has called to inform us that patient just felts fatigued and worn out but nothing unexpected of typical vaccination side effects.

No additional AE reported; the nurse didn't screw the needle tight; the patient got only 1/2 the dose of GARDASIL 9; This spontaneous report was received from a nurse referring to an 11-year-old female patient. Information regarding the patient's concurrent conditions, medical history, historical drugs and concomitant therapies was not provided. On 16-JUN-2022, the patient was vaccinated with with hpv rl1 6 11 16 18 31 33 45 52 58 vlp vaccine (yeast) (GARDASIL 9) injection, lot #U035286, expiration date: 21-OCT-2023, administered for prophylaxis (strength, and route of administration were not provided). The reporter informed that the patient got only 1/2 the dose of hpv rl1 6 11 16 18 31 33 45 52 58 vlp vaccine (yeast) (GARDASIL 9) since the nurse didn't screw the needle tight (underdose and product preparation error). No additional adverse event reported.

No additional AE reported; First dose of GARDASIL 9 was administered on May 20, 2022. Second dose of GARDASIL 9 was administered three weeks later on Jun 3, 2022; This spontaneous report was received from a nurse, concerning a patient of unknown age and gender. Information regarding the patient's pertinent medical history, concurrent conditions and concomitant medications was not provided. On 20-MAY-2022, the patient was vaccinated with the first dose of hpv rl1 6 11 16 18 31 33 45 52 58 vlp vaccine (yeast)(GARDASIL 9) for prophylaxis, lot #U035286 and expiration date reported as 21-OCT-2023 (strength, dose, frequency and route of administration was not provided). On 03-JUN-2022 (also reported as three weeks later) the patient was accidentally vaccinated with the second dose of hpv rl1 6 11 16 18 31 33 45 52 58 vlp vaccine (yeast)(GARDASIL 9) also for prophylaxis lot #U035286 and expiration date reported as 21-OCT-2023 (inappropriate schedule of product administration). No additional adverse event was reported.

No adverse event; 9 year old patient was accidentally administered the second dose of GARDASIL 9 intended for another patient; This spontaneous report was received from an administrator, referring to a 9-year-old patient of unknown gender. The patient's medical history, concurrent conditions, concomitant therapies, drug reactions/allergy were not reported. On 19-MAY-2022, the wrong patient was administered a "booster dose" of hpv rl1 6 11 16 18 31 33 45 52 58 vlp vaccine (yeast) (GARDASIL 9). The patient was accidentally administered the second dose of hpv rl1 6 11 16 18 31 33 45 52 58 vlp vaccine (yeast) (GARDASIL 9) formulation prefilled syringe, 0.5 milliliter, lot #U035286, expiration date: 21-OCT-2023, administered for prophylaxis (strength, and route of administration were not provided), which intended for another patient (wrong patient received product). The patient who received the hpv rl1 6 11 16 18 31 33 45 52 58 vlp vaccine (yeast) (GARDASIL 9) did not have any prior vaccination history with hpv rl1 6 11 16 18 31 33 45 52 58 vlp vaccine (yeast) (GARDASIL 9). No additional adverse event reported.

Symptomtext

The second dose of the Gardasil series was given 3 weeks after the first dose. Merck Co was contacted, due to early timing of the second dose, notified no harm would come to the patient. Merck provided timing for additional dose to be 5 months from the initial dose to complete the series. Pt notified and verbalized understanding.

Patient was given an HPV injection at another office this AM, he was subsequently asked to go the office for what should have been an MCV injection but was inadvertently given another HPV vaccine at that time. He was asymptomatic upon leaving the clinic and a follow up phone call reveals him to be doing well with no ill effects noted.
