Symptomtext

After giving the 3 vaccines, I was by the door leaving the room and I heard the child's mom screaming for help because the patient faint. I turned around and saw patient lying on the floor, he was bleeding from his gums and lips , I hold him while the Dr went into the room with the other nurse. Dr did an examination to the patient vital sign were steady. We put some gauze into child's mouth to stop the bleeding. We gave water to the patient while he was sitting down. We observed him for 1 hour, vital signs were steady and we release him to go home.

Hives appeared the next morning 7/27/2022, responded to benadryl. Hives recurred 7/28/2022 a little more intensely, responded to benadryl and famotidine. No trouble breathing/ talking.

On 05-DEC-2022, the patient was vaccinated with improperly stored dose of hpv rl1 6 11 16 18 31 33 45 52 58 vlp vaccine (yeast) (GARDASIL 9) injection, lot #U036586, expiration date: 21-OCT-2023, administered intramuscularly for prophylaxis (strength, exact dose and dose number was not provided) and influenza vaccine live reassort 3v (FLUMIST), lot #PH3533B, expiration date: 21-OCT-2023, administered nasally for prophylaxis (strength, exact dose, dose number, and formulation were not provided) (Product storage error). On an unknown date, before the administration, the vaccines underwent temperature excursion for unknown period of time. No adverse events were reported (No adverse event). Medical assistant calling to report administered vaccine temperature excursion on multiple patients. Per caller vaccines went out of range yesterday and it was not discovered until this morning when checking the data logger; This spontaneous report was received from a physician and refers to a patient of unknown age and gender. There was no information about the patient's concurrent conditions, concomitant therapies, drug reactions, allergies or medical history provided. On 13-JUN-2022, the patient was vaccinated with a dose of an improperly stored hpv rl1 6 11 16 18 31 33 45 52 58 vlp vaccine (yeast) (GARDASIL 9) pre-filled syringe, lot # U036586 was confirmed to be valid, expiration date reported as 21-OCT-2023, for prophylaxis (exact dose, route of administration and anatomical site were not provided). Vaccine was improperly stored for an unknown amount of time in an unknown temperature. Data logger was used to capture the temperature excursion.
