He developed a small bowel to small bowel intussusception on June 25, 2022 which spontaneously reduced. This occurred in the midst of persistent diarrhea which started the week of his 2nd dose of Rotateq. The diarrhea continued to increase. By June 14, was having diarrhea every 10-15 minutes and vomiting. Seen in ER, no change in plan. Then within a few days developed blood in stool which became more intense and associated with pain and sent to ER where the US showed a small bowel to small bowel intussusception which reduced. Admitted overnight. Mom had been dairy free for months. Subsequenttly bloody stools resolved after elimination of soy from maternal diet as well

healthy infant but hx of premature at 33 3/7 weeks gestation. Stat c/s due to fetal heart rate abnormality. Apgars 3 and 8. Brief resp support but on RA by 1-29-22. Hyperbili and phototherapy. Apnea of prematurity. Was exposed to parents who had COVID19 at the time of his birth. Infant COVID19 negative. Received amp and gent briefly but sepsis ruled out. Immediate post-injection reaction Vomiting

2 month old patient immediately vomited after receiving a dose of ROTATEQ; This spontaneous report was received from a nurse and refers to a 2-month-old male patient. The patient's medical history, concurrent conditions and concomitant therapies were not provided. On 08-JUN-2022, the patient was vaccinated with a dose of rotavirus vaccine, live, oral, pentavalent (ROTATEQ) 2 ml by oral route, lot # U037810, expiration date 28-MAR-2023 for prophylaxis (also reported as routine vaccination). Immediately after receiving a dose, the patient vomited. The outcome of vomiting was reported as recovered. The casual relationship between the event and suspect vaccine was not provided.

Symptomtext

Infant in clinic for first health check. Patient healthy and growth chart shows gaining weight, gaining length. During the health check, I inadvertantly administered required 2 month vaccines to a 22 day old infant. I did not utilize the recommended immunzation schedule for children and adolescent guide before administering the immunizations listed above. Upon my mistake, I contacted the parents reporting the error and to check on the health of the infant. Mother reports no adverse reaction at the time of phone call. Advised to call with questions or concerns.

Symptomtext

The office manager in a physician's office reported that unknown number of patients received improperly stored vaccines.; No additional AEs were reported; This spontaneous report has been received from an office manager concerning an unknown number of patients of unknown age and gender. The patients' concurrent conditions, medical history, concomitant therapies and drug/allergy history were not reported. On an unknown date, the patients were vaccinated with an improperly stored dose of rotavirus vaccine, live, oral, pentavalent (ROTATEQ) (lot # u037810, has been verified to be a valid lot number for rotavirus vaccine, live, oral, pentavalent, expiration date reported and upon internal validation established as 28-MAR-2023) (dose, anatomical location and route of administration were not reported) for prophylaxis. The vaccine was stored at a temperature of Fahrenheit (F) during 6 hours and 59 minutes (Product storage error). The vaccine did not suffer previous temperature excursions. No adverse event was reported.

Symptomtext

Nurse drew up 0.4% Sodium Chloride solution from the ActHIB immunization box and mixed with the Dtap and IPV component from the Pentacel box. Sanofi was consulted. Health Department consulted. Expert Physcian said that DTap and IPV is considered a valid dose. Plan is to give HIB for completion of the Pentacel. Pt arrived on 6/17/2022 and received ActHIB.

Symptomtext

Blood/mucus in stool within 24 hours. Clinically, I presume this is incidental milk-protein induced allergic colitis, but given onset within 24 hours of rotavirus vaccination, I deemed this appropriate to report. Mother breastfeeding and had eaten significant amount of ice cream shortly before onset. Resolved with maternal dairy elimination.
