Pt received vaccines on 23AUG2022; Pt reported back to the clinic 25AUG2022 with a swollen lymph node, red L thigh, and a fever. Pt was prescribed Ibuprofen and Augmentin. Pt was asked to return to clinic 26AUG2022 to see if the issue resolved itself.

No other AE; The caller said the patient had a titer drawn (date unknown) and she needed a dose of MMRII, but the patient inadvertently received PROQUAD; 37-year-old patient; This spontaneous report was received from the Medical Assistant, and refers to a 37-year-old female patient. Her medical history, concurrent conditions, and concomitant therapies were not provided. On an unknown date, the patient was vaccinated with measles, mumps, rubella and varicella (oka-merck) virus vaccine live (PROQUAD) recombinant Human albumin, subcutaneous injection, 0.5 milliliter, lot #u039255, expiration date: 09-JUN-2023, administered for viral titre decreased (strength, and route of administration were not provided); inadvertently, instead of measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live (M-M-R II) (product administered to patient of inappropriate age, wrong product administered). No other adverse event occurred.

no additional AE was reported; was given PROQUAD instead of MMRII; PROQUAD was inadvertently administered concomitantly with VARIVAX; This spontaneous report was received from a medical assistant referring to a 12-month-old male patient. The patient's medical history, historical drugs, concurrent conditions and concomitant medications were not provided. On 24-MAY-2022, the patient was vaccinated with varicella virus vaccine live (oka/merck)(VARIVAX) 0.5 milliliter (route of administration, lot number and expiration date not reported) and inadvertently received measles, mumps, rubella and varicella (oka-merck) virus vaccine live (rHA)(PROQUAD) 0.5 milliliter, lot number U039255, expiration date 09-JUN-2023 (route of administration not reported) for prophylaxis instead of measles, mumps, and rubella (wistar ra 27-3) virus vaccine, live(M-M-R II) (lot number and expiration date not reported) (wrong product administered and accidental overdose). No adverse event was reported.

Symptomtext

No additional AE; a patient was administered PROQUAD after a temperature excursion; This spontaneous report was received from an office manager referring to a patient of unknown age and gender. No information regarding the patient's pertinent medical history, and drug reactions or allergies was provided. Concomitant therapies included sterile diluent (MERCK STERILE DILUENT) (lot number T018038 is an invalid lot number for sterile diluent (MERCK STERILE DILUENT). Lot number T018038 does not match any Company product as confirmed by Lot Verification team). On 21-MAR-2022, the patient was vaccinated with an improperly stored dose of measles, mumps, rubella and varicella (oka-merck) virus vaccine live (recombinant human albumin (rHA)) (PROQUAD) at a dose of 0.5 ml for prophylaxis (lot number U039255 was verified to be a valid lot number, expiration date: 09-JUN-2023; route of administration, vaccination scheme frequency, and anatomical location were not reported) (product storage error). The administered dose of the vaccine experienced a temperature excursion at minus 14 degree Celsius up to minus 10 degree Celsius (minus 10.8 degree Celsius) for 0 hours 45 minutes and 0 seconds, and at minus 10 degree Celsius up to 8 degree Celsius (minus 5 degree Celsius) for 1 hour, 0 minutes and 0 seconds. The vaccine was reconstituted with the sterile diluent, however, sterile diluent was not improperly stored. There were no previous temperature excursions. No additional adverse event (AE) was reported.
