Symptomtext

Patient received listed vaccines on Wednesday 8/23/2023. Vaccines were administered to both the deltoid (IM), as well as to the fatty tissue on the posterior aspect of the upper arm above the triceps (Subcutaneous). Patient presented to Health Dept. on Friday 8/25/2023 complaining of an adverse reaction in the right arm in the area of the deltoid muscle. On assessment, the area presented with diffuse but mild redness. The injection site did not appear swollen, but was firm to the touch. The site was not noticeably warm to the touch. The patient reported no pain on palpation of the affected site, but reported pain at 2/10 when moving the affected arm. The left arm, which also received vaccines on 8/23/2023, present no adverse reaction or complaint from the patient. The subcutaneous site of the right arm also presented without adverse reactions. After assessing the patient, this RN advised the patient and her family that the site should be assessed by a physician. The patient reported not having a primary care provider. This RN stated that if the injection site did not improve promptly, or if it worsened such as becoming more swollen, tender to touch, or the redness spread, that the patient should seek medical attention immediately. Patient and family expressed understanding and departed the Health Department.

Symptomtext

Patient's mother came to Health Department and showed a picture of child's arm. She stated that swelling, redness, and severe itching on right arm occurred on 3/15/23 when child woke up in the morning. Pt's mother took child to ER on 3/15/23 and was informed by ER MD child had a possible allergic reaction to vaccine. Child was given benadryl and prescribed another medication for the reaction but mother is unable to recall name.

Symptomtext

Patient complained of "itching" around injection site. Upon closer examination the area around the injection site was red and warm to touch. Physician was notified and stated to provide an ice pack and continue to monitor in facility. Patient was monitored for any further reaction from vaccination until physician in the building notified parent to use over the counter Benadryl/ ice to help with the reaction. Patient was not seen for further evaluation.

I was given the patient's chart and it was the wrong chart. I thought I was drawing up a Covid Moderna Bivalent for the patient I had been talking to previously in the waiting room. So I drew up the Covid-19 Moderna Bivalent injection and called out her name and she got out of her seat and followed me into the exam room. I asked her about her health issues and wrote that on her chart. I then told her what I was giving her and she said why was she getting another one. I explained that the antibodies wear off after a few months and this particular one was manufactured to be effective against the newer strains and she ok to give. She then asked why she is not getting the other vaccines talked about with the front office and I told her I would go check on that. Then I gave her a copy of her Immunization Registry record and she stated,"This is not my name." At that time I repeated, "This is not your name?" and she said no, it is not! I asked her why she got up in the waiting room when I called out the name and she said "I must have misunderstood you." So, at that time I asked her to wait in the exam room while I went to get the correct chart. I then received her correct chart and drew up the correct immunizations and took them into her room and administered them to her after explaining that I had given her the wrong shot the first time. She asked if it would hurt her and I explained to her that it would not hurt, but she might have a more robust reaction since she had already had a bivalent Covid booster injection given to her in Sept 22

Right after the last vaccine was administered, the patient layed her head down on the desk. At first, she was replying to the father and the nurse talking to her, then became unresponsive.
