Discharge Provider: MD Primary Care Provider: MD Admission Date: 9/12/2022 Discharge Date: Sep 14, 2022 PRESENTING PROBLEM: DKA, type 1, not at goal (HCC) Type 1 diabetes mellitus with ketoacidosis without coma (HCC) HOSPITAL COURSE: 25-year-old female with past medical history of type 1 diabetes who was admitted to the hospital with DKA. Was also found in the ER to have acute kidney injury. After admission to the intensive care unit DKA quickly resolved with gap closure and resolution of AKI. Was also found upon arrival to have COVID diagnosis. Main symptom from COVID illness was decreased appetite but was not having any shortness of breath or other respiratory symptoms. Was felt that DKA was exacerbated from missing dose of long-acting insulin and from COVID illness. Day of discharge, patient stated was not having abdominal pain, was tolerating diet with good appetite. Phosphate day of discharge was low at 1.8. Did give IV K-Phos. Did send with 3 days of K-Phos tablets 2 times a day. Diabetes team Dr. cleared for discharge and said that he is to continue with the same previous insulin regimen. Discharged stable condition.

She received a pneumococcal 23 vaccine on 06/21/2022. 2 days later she began to have left arm pain at the injection site and noticed "band" of redness around the injection site and swelling around lateral upper arm. Pain was a 8 out of 10, aching, with associated tenderness to palpation. Pain improved with Tylenol. No fevers or chills or drainage from the injection site on left upper arm. No inhibition of day to day activities due to pain. 5 days after initial symptoms started, patient had some elbow discomfort which has resolved. she states her pain is 0 out of 10, but becomes a 3 out of 10 when pressing over the injection site, she states the swelling and redness have significantly gone down. Denies shortness of breath, numbness or tingling of extremities, fatigue, malaise. She denies previous vaccine reaction, she does say she has hereditary angioedema.

Symptomtext

Pneumovax 23 (pneumococcal polysaccharide vaccine - polyvalent, PPV23) was administered to the patient at 1714 on 5/31. Called at 1855 for Afib with RVR, SBPs in the 80s and HR 150s. Patient reported to have flushed skin on the BLE and maculopapular rash on chest. Metoprolol succinate 50 mg PO, metoprolol 5 mg IV x2, cetirizine 10 mg x 2, methylprednisolone 125 mg, and famotidine 20 mg IV were administered. Patient's symptoms suspected to be triggered by allergic reaction to pneumococcal vaccine. This morning, 6/1, patient is reported to be stable.

no adverse event; PNEUMOVAX 23 was mistakenly given to a 2 month old instead of Prevnar 13; input error by the prescribing HCP; This spontaneous report was received from other health professional and refers to a 2-month-old patient of unknown gender. The patient's medical history, concurrent conditions, and concomitant therapies were not reported. On 07-SEP-2022, the patient was mistakenly vaccinated with a dose of pneumococcal vaccine, polyvalent (23-valent) (PNEUMOVAX23) 0.5 milliliter, lot #U039928, expiration date: 31-JAN-2024 (strength, anatomical location, and route of administration were not provided) administered for prophylaxis, instead of pneumococcal vaccine conj 13v (CRM197) (PREVNAR 13) (wrong vaccine administered). It was stated that this was not a product confusion but an input error by the prescribing health care professional (Product prescribing error). No adverse event was reported.
