Good morning, Thank you for making the kids check up fun and easy yesterday! Pt. received a few shots and is having a reaction to one of them. Last night before bed we noticed that the injection site was surrounded by big, raised, red blotches. It was sore to the touch and he said it felt like it was burning. I saw on the paperwork that a rash may occur with the MMR/Varicella vaccine, so we decided to see how it was this morning. Today it has turned into one large, bigger red blotch, that is still painful to the touch. I do have pictures from last night and now today if that helps. I'm wondering if I need to take him in or if there is anything I can do at home? He's never had a reaction before so I'm curious what our next steps should be. Thank you! Sincerely

On 08/07/2023 arount 9:00am Medical Assistant administrate expired vaccine to patient. Medical assitant was documenting the administration of vaccine whe Medical Assitant realized that the vaccine had expired on 06/14/2023. Medical assitant report to clinic practice director who contact Merck. Merck has reported that due vaccine was recently expire and properly storage at the right temperature has reported that vaccine still has the same potency and they do not recommend that the patient be vaccinated again but they will leave up to health provider. Patietn did not have any symptoms.

VACCINE ADMINISTRATION ERROR. CLIENT WAS TO RECEIVE AN MMR AND A VARICELLA. NURSE INADVERENTLY ADMINISTERED A VARICELLA IN THE RIGHT ARM AND A PROQUAD (IWHEN IT SHOULD HAVE BEEN A MMR) IN THE LEFT ARM. THUS THE CLIENT HAS HAD TWO DOSES OF VARICELLA TODAY. NO KNOWN ADVERSE AFFECT AT THIS TIME.

The patient's concurrent conditions, pertinent medical history, historical drugs, and concomitant therapies were not reported. On an unknown date, a measles, mumps, rubella and varicella (oka-merck) virus vaccine live (recombinant human albumin) (PROQUAD) (rHA) vaccine (lot number U039957 has been verified to be a valid lot number for measles, mumps, rubella and varicella (oka-merck) virus vaccine live (PROQUAD), expiration date reported and confirmed as 14-JUN-2023) suffered a temperature excursion of -4.5 Celsius degrees during a time frame of 3 hours 58 minutes. The vaccine had not suffered a previous temperature excursion. On 02-JUN-2022, the patient was vaccinated with a dose of improperly stored measles, mumps, rubella and varicella (oka-merck) virus vaccine live (recombinant human albumin) (PROQUAD) (rHA) (lot number U039957, expiration date 14-JUN-2023) (dose, route of administration, and anatomical location were not provided) once, as prophylaxis (Product storage error). 5/2/2022 @ 1430 an incomplete MMRV vaccine was administered. Client jerked and pulled away and the full 0.5 ml was not injected. Contacted public health office Dr. and informed him of the incident. Per Dr. - administer another 0.5 ml dose since we can not be sure how much of the original dose was actually administered. Second dose administered 5/2/2022 @ 1450. Client monitored for adverse reactions, none to report. Parent/Guardian of client instructed to contact EMS if a severe reaction is witnessed.
