Dizziness Immediate post-injection reaction Loss of consciousness

Symptomtext

Patient felt lightheaded immediately after receiving Japanese Encephalitis vaccine. This vaccine was the fifth and last vaccine given on 12/28/2022. Patient received vaccine at 4:07 PM and immediately felt lightheaded. Patient was seated on exam table and began to pitch his body forward. Patient was able to lay back on exam table with his feet up. Patient did not lose consciousness and continued to speak with this RN. Patient was provided with a cool compress for his forehead. Patient remained supine for 10 minutes and then patient reported he felt better and was able to sit up and walk without experiencing any dizziness.

Symptomtext

Patient received both the Flu and Typhoid shots in the left arm. After administering the Rabies shot in the right arm the patient fainted in her seat and was unconscious for ~30 seconds before recovering on her own. Patient did not want to call EMS and sat in the waiting area for about 30 minutes and spoke with her primary care provider over the phone before leaving. Patient was not displaying any other symptoms during that time period.

Symptomtext

Patient received the three vaccinations listed above around 6:25PM. I requested patient sit in the waiting area following immunization for about 15 minutes to be monitored. Within 5 minutes of sitting down patient appeared to have briefly convulsed then passed out. I yelled her name then exited the pharmacy and went to her at the chair. I shook her and she came to very quickly. I provided patient with water and an ice pack. She remained awake and oriented to time and place for the next 20 minutes. She expressed after this time that she was okay and was comfortable leaving to go home/meet her ride. I called patient the next day to follow up and see how she was feeling but was unable to reach her.

Symptomtext

The patient was due for second dose of adult Hepatitis A (Havrix) vaccine. Patient was inadvertently given a Twinrix dose. Patient was then given a pediatric dose of Hepatitis A (Havrix) on the same day, to equal a full adult dose of Hepatitis A vaccine. Patient was informed that if she wants to complete her three-dose Hepatitis B series, she would need two additional doses of Engerix-B or Heplisav-B. Patient denied any symptoms, complications, or adverse effects from the vaccine.

Symptomtext

It was determined there was a preparation error at Facility on 1/19/23. Between 4-5 individuals received Moderna 6 month to 5 years monovalent formulation. Preppers unintentionally prepared 1 vial of the 6 months to 5-year monovalent formulation (with volume of 0.5 ml per dose) and labeled it as a Moderna 12+ years bivalent formulation. There was a total of 14 patients scheduled to receive the Moderna 12+ years bivalent on 1/19/23. Clinic is unable to determine if patient received the correct bivalent dose versus the Moderna 6 month-5 years monovalent formulation. Information about other Moderna formulation: Moderna 6 months to 5 years monovalent dose, Lot AS1414B, manuf. exp 2/13/23. Patient has been notified of vaccine error that may have potentially impacted him. No reported signs/symptoms of adverse reaction.
