pt is stating that her arm is still sore 1 month after vaccine, and not full range of motion. She came in today 2-3-2021. I suggested that she put heat on area and try aleve/ or ibuprofen for a few days and see how i is. I will follow up with her in a few days

Symptomtext

Pt administered TDAP/Menactra/HPV administered IM tolerated well. Pt or Mother denied problems or concerns at present time. While I washed my hands upon completion of administration, the pt stood beside my desk and stated she felt like she was going to vomit. I immediately handed the pt the trash can in case vomiting occurred. Pt gagged twice but did not vomit. I applied a cool wet towel to the patients forehead. Pt then states she felt "numb" and went limp. Possibly experiencing a vasovagal syncope episode. Pt was assisted to sitting position by support of myself and her mother. Pt remained conscious and answered questions appropriately when asked by pt's mother and nurse. Mother denies occurrence happening in the past. Pt assisted to lying position on exam table while in nurse's room. B/P at this time 116/68. Pt no longer had flushed appearance and states she feels better. Mother states they did not eat before coming to the clinic and they planned to eat as soon as they left. Pt issued a bottle of water and animal crackers. Tolerated well. Pt alert and oriented. Denies any problems or concerns, stated she feels much better and she did not know why that happened. Pt reassured and informed that syncopic episodes do occur with needle sticks or injections at times. Verb an understanding. Respirations even and unlabored. Instructed to wait 20 mins for observation. Pt waited with mom and nurse. Pt' skin warm and dry to the touch. Respirations even and unlabored. Ambulating with ease. Skin normal appearance for race. Left with mother after observation period. Denies any problems or concerns at present time.

This case involves an unknown age and unknown gender patient who experienced patient was given expired tdap, with no reported adverse event, while receiving vaccine DIPHTHERIA-2/TETANUS/5 AC PERTUSSIS VACCINE [ADACEL]. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. On an unknown date, the patient received a dose of suspect DIPHTHERIA-2/TETANUS/5 AC PERTUSSIS VACCINE suspension for injection lot U6518AA with expiry date (11-May-2021) (unknown dose, formulation via unknown route in unknown administration site) as prophylactic vaccination. On an unknown date, the patient developed a non-serious event "patient was given expired tdap, with no reported adverse event "(expired product administered) (latency same day) following the administration of DIPHTHERIA-2/TETANUS/5 AC PERTUSSIS VACCINE. expired dose of ADACEL was administered with no reported adverse event; Initial information regarding an unsolicited valid non-serious case was received from other health professional via Consumer/ Other Non Professional via Global Medical Information (Reference number- SANOFI) and transmitted to Sanofi on 11-Nov-2021. This case involves a 11 years old female patient who received expired dose of DIPHTHERIA-2/TETANUS/5 AC PERTUSSIS VACCINE [ADACEL]. Past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included MEASLES VACCINE LIVE (ENDERS-EDMONSTON), MUMPS VACCINE LIVE (JERYL LYNN), RUBELLA VACCINE LIVE (WISTAR RA 27/3), VARICELLA ZOSTER VACCINE LIVE (OKA/MERCK) (PROQUAD) for Immunisation. On 11-Nov-2021, the patient received a 0.5 mL dose of suspect DIPHTHERIA-2/TETANUS/5 AC PERTUSSIS VACCINE lot U6518AA, expiry date 05-Nov-21 via intramuscular route in the left deltoid for prophylactic vaccination. It was a case of an actual medication error due to expired product administered (latency same day). It was reported "Caller is looking for information on possible adverse effects as well as if they need to revaccinate the patient." At time of reporting, no adverse event was reported. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.
