Symptomtext
Post vaccination syncopal episode immediately following injections and lasting a couple of seconds. Pt quickly recovered and was given apple juice and two lollipops and returned home. Menactra and Adacel were administered during the patient's routine well child check. VS prior to injection (5/17/21) T-98.3 P-73 R-18 BP-108/60 SpO2 98% on RA Ht: 59.2 inches Wt: 101 lbs. Pt's father called today reporting pt was sent home from school with c/o dizziness, nausea, headache and temp 99.2-F (5/18/21 11:50AM). Encouraged rest, fluids, APAP per mfg directions, continue to monitor.
