Symptomtext

Trumenba was admin to the pt L deltoid w/ no voiced complaints @ approx 2:20pm. Pt remained alert. About 1-2 min later the #2 Menaetra vaccine was admin to the pt R deltoid w/ no voiced compliments. Moments later, about 1-2 min following the injection the RN noticed the pt's eyes close & her head began leaning forward. Pt become unresponsive to voice or touch to the shoulder. The pt's eyes closed & her head & eyes rolled backward. Pt began to have severe like activity w/ extraocular eye movements w/ upper lower extremity muscle retraction which lasted about 15 seconds. RN immediately inspected The pt & her surrounds to minimize risk of injury. Pt regained conciousness. Pt was notably disonented & hypotensive w/ a manual pressure of 70/30 HR 52 + Sp02 99% on mm air. Ems called. No head or tongue injuries sustained. No loss of bowel or bladder functions. Dr alerted & came to eval the pt. Pt given Po fluids and instructed to remain seated until EMS arrived. Pt transported in stable condition.
