1st adverse event: was chest congestion with bad coughing and hard time breathing. Called 08/26/21 to clinic, prescribed over the phone high dose of prednisone for 2 weeks. Within 2 weeks after, started having muscle cramps, painful muscle tightening and stiffness all over my body. Had very hard time working. Saw another doctor, after blood test, found out that I had high c-reactive protein level at 70.8, sedimentation level at 48, then was referred to the rheumatology dept. at clinic. After many more blood test, Ct's, X-rays, I was diagnosed with Polymyalgia Rheumatica.

The same night that the immunizations were given mom thinks that was when symptoms started and then child confronted mom the next day and his arm was red, triple in size, warm to touch, itches, and hard knot. Mom states that whole top arm is huge, hard and swollen. Mom has been given Benadryl and Ibuprofen and ice packs.

Patient stated she had no symptoms until the next morning when she awoke to soreness at right deltoid. Patient stated she looked at her arm and saw approximately a 6 inch in diameter firm nodule that was hot and red at the vaccination site. Today the nodule is approximately 3 inches in diameter and firm.

expired Adacel (03Jul2022) was administered to a patient with no reported adverse event; Initial information received on 05-Jul-2022 regarding an unsolicited valid non-serious case received from a other health professional. This case involves a 81 years old female patient and it was reported that expired Adacel (03-Jul-l2022) was administered to a patient with no reported adverse event , and the vaccine involved was DIPHTHERIA-2/TETANUS/5 AC PERTUSSIS VACCINE [ADACEL]. The patient's past medical history included laceration to her foot (Skin laceration). The patient's past medical treatment(s), vaccination(s) and family history were not provided. On 05-Jul-2022, the patient received 0.5 ml dose of suspect DIPHTHERIA-2/TETANUS/5 AC PERTUSSIS VACCINE, (lot: U6706AA ; expiry date: 03-Jul-2022) via intramuscular route in the left deltoid for immunization. It was reported that patient came to the clinic on 05-Jul-2022 for a tetanus shot after a laceration to her foot while she was inventorying the clinic vaccines. She had not yet gotten to the ADACEL, and it was still on the shelf for someone to inadvertently give. On the same day, it was later noted that , an expired Adacel (expiry date: 03-Jul-2022) was administered to a patient with no reported adverse event (expired product administered). Vaccine was noted to have expired 2 days ago. Treatment would be repeated Action taken : not applicable. Outcome: unknown This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the guidelines. Pt was given the tdap and menveo in the right deltoid, pt was given the flu and HPV in the left deltoid. Pt mom brought him back in the next day with the right arm red and hot to the touch. pt was seen by the provider and she prescribed antibiotic cephalexin 500 mg three times daily for one week and loratadine 10 mg daily she advised him to ice it and marked redness with pen
