Pt became diaphoretic, vasovagal reaction loss of site for less than a minute occurring 10 minutes after vaccination administered in waiting room, nausea, SOB. BP 100/62, Pulse 77 4 minutes reading. EMS called Paramedics arrived pt examined and reported at 19 minutes after to be stable conscious with slight dizziness. Pt was initially placed on exam table supine with legs elevated.

Grandmother verbally reported to that pt. "fell and fainted". When staff looked up at pt., she was returning to a sitting position from leaning to the left in the chair. No fall or fainting witness by the staff. When I approached pt., she was sitting in the chair. Pt. verbally reported that her head was hurting on the right side of her head and that her arm was hurting on the right side where she received the vaccine. Temperature taken and was 97.3, B/P 107/73 on left arm, RR 20 and HR 80. Staff translated in other language with the grandmother and she reported that pt. never had a reaction to vaccines in the past and that pt. had eaten breakfast. Pt. was encouraged to continue to drink fluids. Pt. was reassessed every 5 minutes and verbally reported that she was feeling better, head and arm was hurting less and that she felt back to normal. After the 30 minutes of observation, pt. was able to stand, lift arms above head, and she ambulated around the chairs in the post vac area without any signs of dizziness and no verbal complaints. Patient left the post vacc area without ambulating independently and verbally stating that she felt back to normal. Post vacc instructions translated to the grandmother from staff.

At 0930 on 6/15/2022, completed administration of immunizations to client (order Right Arm: Hep A, MenQuadfi, Left Arm: Tdap, HPV). After five seconds, client's skin color became pale, lip color turned blue, eyes rolled into head, and client lost consciousness. Client unconscious for thirty seconds before awaking. Client alert but oriented to person. Client anxious, crying. Assured client he was safe and brought mother around into client's vision better. Reclined chair back 45 degrees. Used cool compress to the temples. After five minutes of mother talking and providing touch, client stopped crying and became oriented x 4. Client stated felt dizzy and nauseous. RN gave client some time but also outlined for mother and client goals for client to drink, eat, and be steady on feet before client could leave clinic. Provided water and Graham crackers. After five minutes, client sat forward on own in chair and able to tolerate water and graham crackers. Skin color pink, lip color pink, behavior almost returned to baseline. After five minutes, client wants to attempt standing. Client stood and took five steps in room; client stated feeling dizzy. Returned client to reclined chair, encouraging client to sip water. After ten minutes, client attempts standing again. Client able to tolerate. RN and mother escort client to private vehicle.

About 10 minutes after immunizations administered patient was standing at check out and told mom he didn't feel right. He sat down on bench and mom reports patient convulsed, b/l arms and head x2 for about 3-5 seconds. Patient thinks he may have blacked out momentarily, but recalls full event. Patient then reported he felt normal, but appeared pale. He stayed in lying position for at least 5 minutes with cool compress to forehead/neck, patient's coloring returned to normal. He then sat for 10 minutes, drank 2 cups of water continued to report he felt fine. Denies headache, dizziness, lightheadedness or ringing in ear. HR was 60 bpm, RRR. Patient walked around waiting area, felt fine and left the office with mom. Reviewed with mom concerns to monitor for.

Mother assisted patient to ground; patient did not sustain any obvious physical trauma. Patient alert and oriented within seconds of episode. Transported via EMS for evaluation/monitoring. Patient developed dizziness, severe right arm pain and heaviness, hyperventilation, severe shivering, diffuse "pain", nausea, and altered level of consciousness over the course of 20 minutes post-vaccination. Was unable to lift/use right arm and any movement/palpation of right arm caused shivering/tremors and pain. Vital signs including BP, HR, temp, and Oxygen sat remained stable throughout. After 20 minutes pt became very fatigued and was difficult to arouse intermittently therefore 911 was called and paramedics arrived, VS remained stable, blood glucose 95, IV started, pt was given Benadryl IV and transported to ED for ongoing evaluation and observation. Per parent, within 45 minutes of arrival in ED pt had fully recovered, symptoms had resolved completely including pain, and pt was discharged home with diagnosis of "panic attack"

The patient received the above 2 vaccines at 10:03 AM. She felt dizzy and nauseated about 8 to 10 minutes later. Blood pressure was 112/87, pulse rate 76 at 10:13 AM. Heart rates were regular sinus rhythm with no heart murmur. She then went to the rest room where she vomited and then had diarrhea. A follow-up check at 10:26 AM showed BP 119/96, Pulse rate 85. A few minutes later she left the office to go home because she felt much better.

Patient started to feel unwell while out to dinner on 7/15/2022. States that his head "felt like it could explode", Went home and that it when mom called the nurse triage and was told to have him seen at the hospital or to call an ambulance. Patient then reported at the ED that he was confused with tactile fever and could not remember the events of the day. Felt foggy and nauseated. Labs and IV were provided at the ED and everything was normal at time of discharge. Patient did experience nausea and diarrhea the next morning.

Attempted to sit her up continued c/o dizziness and if standing blurry vision. After 15 min called her father to come and get her. 15:10 Hr 74 o2 sat 98. Recommended going to ER for evaluation. Father did not want to take her to ER. Will call to follow up tomorrow.

This case involves an unknown age female patient who experienced short term memory loss for three days, headache and fatigue while receiving vaccine MENINGOCOCCAL A-C-Y-W135 (T CONJ) VACCINE [MENQUADFI]. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. On an unknown date, the patient received a dose of suspect MENINGOCOCCAL A-C-Y-W135 (T CONJ) VACCINE (lot U6821AA, expiry date: 27-Jun-2023, anatomical location: unknown) as prophylactic vaccination. On an unknown date the patient developed a non-serious "short term memory loss for three days (amnesia)" (unknown latency) following the administration of MENINGOCOCCAL A-C-Y-W135 (T CONJ) VACCINE. On an unknown date the patient developed a non-serious "headache" (unknown latency) following the administration of MENINGOCOCCAL A-C-Y-W135 (T CONJ) VACCINE. On an unknown date the patient developed a non-serious "fatigue" (unknown latency) following the administration of MENINGOCOCCAL A-C-Y-W135 (T CONJ) VACCINE. Persistent headache x 3 days,. Bitemporal headache developed the morning after receiving the vaccine and is associated with photophobia. The patient has treated with headache with OTC tylenol and ibuprofen with minimal improvement. She has not had any periods of time without headache. No blurred vision, no double vision. This case involves a 11-year-old male patient who experienced He (patient) was acting funny, quiet, and not himself (Behaviour abnormal), red welts (Urticaria), was weepy and crying a lot, pale (Crying), pale (Pallor), lethargic (Lethargy), congestion (Nasal congestion), nose was swollen and inflamed (Nasal inflammation), threw up (Vomiting), lesions on his elbow on the same arm (left arm) as the injection, they were like little pimples (Dermatitis acneiform), it was like pus-like ulcers and they started to swell (ulcer) while receiving MENINGOCOCCAL A-C-Y-W135 (T CONJ) VACCINE [MENQUADFI]. Medical history, medical treatment(s), vaccination(s), concomitant medications and family history were not provided. On 25-Aug-2021, the patient received 0.5 ml (once)dose of suspect MENINGOCOCCAL A-C-Y-W135 (T CONJ) VACCINE [lot U6821AA and expiry date not reported] via unknown route left arm for prophylactic vaccination. On 25-Aug-2021 the patient developed He (patient) was acting funny, quiet, and not himself (Behaviour abnormal), red welts (Urticaria) (latency on the same day), on 26-Aug-2021 the patient was weepy and crying a lot, pale (Crying), pale (Pallor), lethargic (Lethargy), congestion (Nasal congestion), nose was swollen and inflamed (Nasal inflammation), threw up (Vomiting) (latency 1 days) and in Sep-2021 lesions on his elbow on the same arm (left arm) as the injection, they were like little pimples (Dermatitis acneiform), it was like pus-like ulcers and they started to swell (ulcer) (latency 2 weeks) following the administration of MENINGOCOCCAL A-C-Y-W135 (T CONJ) VACCINE. Other relevant tests were not reported. The patient was treated with DIPHENHYDRAMINE HYDROCHLORIDE (BENADRYL ALLERGY [DIPHENHYDRAMINE HYDROCHLORIDE]). They (doctor) gave him a topical steroid. They didn't do a biopsy on the welts. They said that they would do a biopsy if they kept coming. They (lesions) left some scarring. At the time of report the outcome of event was unknown.

Within 1.5 hours of receiving the vaccine, pt. was tired and quiet. Personality was changed. He also had a red swelling on his left arm (not at injection site but same arm. )A few hours later I noticed the red swellings on his legs at well. The next day (Thursday) he was lethargic and weepy. Friday he woke up very pale and lethargic, he also had severe congestion. This went on a few days . Monday he had a severe heartburn like pain and threw up. Wednesday he seemed to feel better but was still tired. The following monday he began developing ulcer like lesions on his left elbow. They were about penny sized, oozing and rapidly spreading. By the following week they began to appear on his right elbow. He was taken to both his Doctor and dermatologist,, The Dermotologist said it was inflammatory reaction. He prescribed a steriod.

This case involves a nurse called to inform that they have given an expired meningococcal a-c-y-w135 (t conj) vaccine [Menquadfi] to a 17 years old male patient with no reported adverse event. Client presented to clinic for seventh grade vaccinations on 6/13/2022. At 11:40AM, client received vaccines listed above. Twenty seconds after completing vaccine administration, client stopped talking mid-sentence and leaned against cushion of exam table. Eyes staring ahead unfocused, client not responsive to verbal stimuli. After four seconds, client spontaneously sat up, becomes awake and alert x 4, and commented "I must have fell asleep for a moment" then continued with conversation. Lip color pale (client skin color very fair as baseline). Mother stated client has never passed out to vaccines before. Mother stated client admitted not have breakfast before coming. Client also stated attended camp all week the week before and "still sleep deprived." Mother stated family history of father passing out routinely for vaccines and blood draws. Gave water and Goldfish crackers, able to tolerate. After five minutes, client stated felt good to go. Escorted out to private vehicle by mother.
