Vasovagal response. Skin color became pale, white lips, but was still able to speak with family and nurses. Pt was shaking, diaphoretic, and rap pulse on assessment. Pt stated, "my ears started ringing, bad pain in my left arm, things got dark, and remembering sitting on the floor when I came to." 911 was called. After deep breathing exercises and cool compresses pt slowed breathing, pinks became pink, and blood pressure/HR came back in normal range for his age. O2 sats were 98%.

Shortly after receiving his vaccines (within 30 seconds to 1 minute), patient stated that he felt like his vision was blurry and like he was going to pass out. He became very pale. We got him some water. He drank a few sips, then slumped forward on the exam table (2-3 minutes after vaccine administration). We laid him back and he regained consciousness within 10 seconds. He was still pale and diaphoretic. He drank more water and ate a snack while we watched him and his vital signs. They remained stable. After around 20 minutes, patient stated that he felt much better. His color returned and he was no longer diaphoretic. He left the office with no further incident. No sign of any allergic response, such as rash, vomiting, angioedema or difficulty breathing. Discussed with patient and father that syncope following medical procedures is extremely common, especially in adolescents. This is not exclude patient from other vaccine administrations and is not an allergic reaction.

Pt received vaccine at 2:10pm. Patient was receiving HPV9 for the first time, so patient was told to wait 15 minutes to make sure he felt OK. At 2:18pm patient fainted. Elevated patients leg while lying on the ground. BP obtained 88/60 O2 sat 99 HR 64. Patient started to come to approx 2 minutes after fainting. He began to talk and drank a sprite. BP obtained again at 2:30pm 100/60. Patient stated that he felt much better. Patient left facility at 2:40pm. Notified pt's mother the following day to check on patient. She stated that patient was totally fine.

Ice applied on forehead and neck for approximately 5 minutes. Alert, oriented. Student was placed on the chair, pale and complained of dizziness. Placed on the ground and 911 was called.

Patient started to faint, pupils looked dilated and she was sweating and her palms appeared blue. I administered benadryl in case she was having an allergic reaction. Patient was breathing, had a pulse and we sat her down and she started to wake up. Her oxygen and blood pressure were normal and she had normal color after 15 minutes. We iced the back of her neck and she started feeling better. She was able to drink water, and eventually had a pretzel with salt. She walked around the store a little and then was fine and left. Probably 30 minutes total

Patient appeared well and responded when spoken to after vaccines were given. Stickers were given to the pt and the pt's sibling. Reviewed the immunization record and VIS with parent. I directed them to the front lobby. A code blue for the second floor lobby was called over the intercom minutes later. I reponded along with several other employees/nurses. I saw that the code was called for this pt. When I arrived to the scene. Security was supporting the pt's head on the floor, and the mother was at the pt's feet with the pt's sibling. The mother reported that the pt told her he felt dizzy, and then he began to fall. She stated that she was able to catch him and lower him to the ground. RNs had arrived at the scene with the crash cart and glucometer. RN was checking the pt's pulse. RN checked the pt's blood suger level as well as respirations. The pt was alert and responded when spoken to. His face and lips appeared to be pale. Pt took small sips of water. I got an ice pack for his head. Pt appeared to be slightly sweaty. We stood there for several minutes. After his color came back and he stated that he was feeling better, he was assisted to a wheel chair. He was taken back to one of the clinic rooms to be monitored by myself. The pt was given more water and some crackers. After monitoring for several more minutes, the pt reported that he felt well. He stated that he felt completely normal and denied dizziness. I assisted him to stand up slowly. RN and myself walked with the pt down the hallway, in the elvator, and to the car. Pt stated that he still felt well. Mother was instructed to follow up with the pt's PCP and to inform him of this situation and the vaccines given today. Mother was also advised to call EMS or go to the emergency department if she notices anything out of the ordinary. Parent verbalized understanding.

Patient was vaccinated At 2:20 pm per protocol and monitored for 10 minutes for any adverse reaction. At 2:30 patient had a mild reaction, some erythremia down left the arm and swelling at the injection site. Mom was present verified allergies again with mother, and then gave patient 4 tablets of children's Benadryl based on patient's weight @ 2:31 pm. Checked vitals at 2:31 pm vitals were stable and remained stable BP: 116/87 Pulse: 72 Temp: 97.9 2:40 pm BP: 118/85 Pulse: 72 Temp: 98 2:45 pm BP: 117/87 Pulse: 74 Temp: 98 No respiratory distress noted. Patient was assessed at 2:50 pm. Patient did not have any difficulty breathing, as well as no other redness or swelling on the body or extremities. Instructed to patients mom if condition got worse to go to the nearest ER. Patient was checked out and discharged with no other problems.

Administered by MA, Patient tolerated okay, she complained of lightheadedness and feeling nauseated. Patient was given apple juice and then patient was laid down. RN came in to room and gave pt a cold compress evaluated her. Patient was diaphoretic and pale. Symptoms improved quickly. Patient drank more apple juice waited about 5 min and then color started to come back. Patient was able to sit up and stand for a few minutes. Denied being dizzy or lightheaded. Patient grandma took patient home.

Patient came in for a 1 month office visit on 10.7.21, patient was due for Hep A, Mennigococcal, and Tdap vaccines. Patient was given Hep A, Mennigococcal, and Dtap vaccines. It was discovered today, 10.15.21 that she was given the wrong vaccine (Dtap instead of Tdap), I first contacted the health department and they informed me patient would need the correct vaccine charted, I spoke to our VFC rep, and she informed me that is not the case, I will need to make a medication error report per Health center's policy and make a report to VAERS. That the patient was just given a stronger dose of diptheria than that is required for patients age. The tetanus part of either vaccine, Tdap or Dtap is the same strength/concentration, so patient is still covered. I informed pateint's mother of the incident. RN

Patient missed HPV and Tdap vaccine/ NO AE; Patient received only half of the MENACTRA dosage/ NO AE; Initial information regarding an unsolicited valid non-serious case was received from the consumer or non-healthcare professional via Medical Information (Reference number: 00793014). This case involves 12-year-old male patient who jump away/ jerked and run off when he was trying to receive a dose of MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE [MENACTRA] about only half the dosage was administered (underdose) and dad was supposed to bring him back but he missed his appointment. He was supposed to get his DIPHTHERIA, TETANUS AND ACELLULAR PERTUSSIS VACCINE and HPV VACCINE but missed those (product dose omission issue). The patient's medical history, past medical treatment, vaccination, concomitant medication and family history were not provided. On 21-Sep-2021, the patient received a total dose of suspect MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE [frequency: once, dose: 0.25 mL to 2 mL for vaccines or 0.1 mL for tuberculin skin test, strength: standard, lot number: U7087AA, expiry date: 05-Oct-2022] via an intramuscular route at an right deltoid for prophylactic vaccination. It was an actual medication error case due to Vaccine underdose and missed dose (latency: same day). No other vaccines. It was reported " Office manager states they had a patient jump away and run off while they were trying to administer a dose of MENACTRA to the patient. Caller states about only half the dosage was administered. Caller states he was supposed to get his human papilloma virus (HPV) and combined tetanus, diphtheria and acellular pertussis (Tdap) but missed those". Caller would like to know if they need to fully revaccinate or if there is another way to move forward. At the time of reporting, no adverse event was reported. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.

First vaccination was given on July 29th and the second was given on August 5th with no AE; Initial information regarding an unsolicited valid non-serious case was received from other health care professional and consumer/non-health care professional via Global Medical Information (Reference number- 00723783) and transmitted to Sanofi on 13-Aug-2021. This case involves 12-year-old (unknown gender) patient who was given MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE [MENACTRA] first vaccination on july 29th and the second was given on august 5th (extra dose administered) Relevant medical history, medical treatment(s), vaccination(s) concomitant medication and family history were not provided. On 29-Jul-2021, the patient received first 0.5ml dose of suspect MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE (lot number: U7087AA and expiry date was not provided) and on 05-Aug-2021, the patient received second dose of 0.5ml of same vaccine (lot number: U6920AB and expiry date: 15-Apr-2022) via unknown route in unknown administration site for prophylactic vaccination (extra dose administered) It was reported that,"Medical assistant stated that they had a set of twins to come in and that one of the twins was over vaccinated with Menactra. MA (Marketing authorization) is calling to see if there is any concern or what should be done at this point because they want to be certain that the patient is going to be ok. Spoke with office staff who states she is in another office and unable to transfer me to that office but will take a number to have her contact us back. Informed office staff information would be emailed to assist with answering the question". This case was an actual medication error due to extra dose administered (latency: same day) At the time of reporting, no adverse event This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. After receiving the HPV vaccine (HPV last vaccine given following the mcv4/tdap. RA) patient twitched once with arm once the neck then leaned into nurse's shoulder. In and out of consciousness for aprox 3-5 minutes. diaphoretic and loss of some color to lips and face. patient awoke and was alert to surroundings x3.
