Symptomtext

Pt felt a headache, diaphoretic, and short of breath approximately 30 minutes after receiving vaccines, then got dizzy and felt weak; while his friend was getting a wheelchair he convulsed and became unresponsive then confused. Was taken to ED after this episode.

patient stated to boyfriend was seeing black spots so sat down and then lost consciousness. boyfriend said she was shaking for a bit. and then came back around. patient lost all color to face and was sweaty. black spots in vision continued for 10 minutes tingling in fingers continued for 10 minutes. medic came and checked her out and she walked out about a half hour after the shot

Symptomtext

Patient came to pharmacy to get meningitis shot for college. Upon getting the vaccine, patient became unconscious while sitting on the chair. About 10 seconds later, patient woke up and felt nausea. Patient didn't vomit. Patient sat and relax for the next 10 minutes and felt completely before leaving the pharmacy

Pt received vaccine IM on 08/25, Mother called and reported on 08/26 that patient woke up with a swollen right upper arm, .hard to touch and , warm at the injection site, also noted /and reported by parent that right lower arm is also kind of swollen. Pediatrician saw patient today. Will RTC tomorrow for follow up check up.
