Client is 18 year old. She presented in clinic with her uncle, but came back into the treatment room by herself, since she is 18. Records were shown at registration that indicated client had had one dose of Covid-19 vaccine 9/13/21. When discussing with the client the vaccines she would need to enter school, staff commented that she had not completed the Covid vaccine series and asked if she would like her second dose. Client responded affirmatively, and another dose was given to the client. When client returned to her uncle in the waiting room and asked to stay for the 15 minute waiting period, her uncle asked her why she got another Covid shot when she'd had a second dose in October. The client said she didn't realize, and the uncle then produced a covid vaccination card with two vaccines recorded on it in the client's name: the one given, and another given 10/14/21 at a local pharmacy. This card was not presented at the time of registration, and the client admitted she had forgotten about the vaccination. Client waited 20 minutes after receiving her vaccinations, did not complain of or appear to experience any adverse effects. Client was instructed to call 911 or go to the ER if she felt any symptoms of anaphylaxis. Client's younger siblings returned to clinic the following week and uncle did not report any significant reaction/side effects in 18 year old niece. VAERS report made according to CDC guidance.
