Syncopal event he has had this occur in the past with every vaccine- he was administered the vaccines while lying on the exam table- he did initially pass out- feel nauseous- seemed ok after 5 minutes and tried to sit up only to become dizzy again and need to lie down- he was given ice pack- ammonia inhalant used x 1- he had several attempts at sitting up but became dizzy and nauseous- needed to lie down- he tried to drink orange juice and then some gatorade- vomited a small amt after sleeping for 20 minutes he finally felt well enough to stand, walk and go home total time at least 60 minutes from vaccine administration until he was able to walk out the door

Symptomtext

Patient developed nausea, dizziness, and diaphoresis within two minutes of injection. Patient was assisted to side-lying position and ice pack placed on head/neck, wet washcloth to face. No vomiting, no syncope. Patient returned to baseline within 10 minutes. Declined offer of juice and pretzels. Recommended to RTC should they experience any other symptoms.

Symptomtext

Came to appointment for 2nd Men B vaccine on 4/28/22 and reported that 2 hours after receiving vaccines listed in section 17 on 3/24/22 pain, redness, and swelling occurred in bilateral arms down to about the mid bicep. Symptoms lasted about 36 hours and was treated with Tylenol and rest. Denied rash, hives, or itching. History obtained per mother and patient and are reporting a full recovery.

Symptomtext

Mother called the next day after shot and said that it was swollen and red. The area was sticking out almost as much as his shoulder. Instructed mother that he should go see a doctor or if redness or swelling gets bigger or he can't move his arm or has severe pain go to ER. Mother said she was waiting for their doctor to call them back today. I strongly encouraged having it physically looked at by a doctor and she said she would wait.

Dizziness, headache about 15 mins after vaccine administration. Had pt lay down, gave some apple juice. Pt reported having not eaten yet this morning. Vitals: BP 94/63, P 109, R 20, SPO2 97% RA. Pt assessed by Doctor. Pt started to feel better about 10 mins after lying down and given apple juice.

Symptomtext

After child received second IM injection she fell forward from the chair bench to the carpeted floor. No resp. difficulties noted, family at child?s side, child unresponsive few seconds, I got my director to come and assist, child rolled from face down to back. Cool cloth applied to forehead, child noted with 3cm x 0.2cm red area noted to right forehead with no raised area noted and 0.2cm circular red area noted to upper bridge of nose. Legs elevated. 3:00pm B/P 90/40 in laying position with feet elevated and responding to verbal stimuli and alert denying pain or discomforts at this time. At 3:15 child assisted to sitting position child C/O feeling nausea child assisted back down to floor and feet elevated with cool cloth applied to forehead and ice pack placed on back of neck. B/P 60/40. Child continues to respond verbally denying N/V or headache and dizziness at this time. 3:20 B/P 64/40 continue to lay with feet elevated with cool cloth and ice pack. 3:25 B/P 90/50 child assisted to sitting position without any c/o of N/V/HA or dizziness noted. 3:30 B/P 92/52. Child assisted to standing position slowly with no dizziness noted. Child walked out to family car with no difficulties continue to deny pain or discomfort .Father faced time with child during episode father was obviously upset and kept asking the director why she was not sitting at time after vaccine, which child was sitting and fell forward from chair bench. Family member at child side entire time. Medical Director called and updated regarding child. Childs primary physician notified of the incident which was written on our forms. That doctor denied ever seeing this child as a patient.

This case involves an unknown demographic and unknown gender patient who experienced is reported as potential ae because of temperature excursion while receiving vaccines IPV (VERO) [IPOL] and MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE [MENACTRA]. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. On an unknown date, the patient received a dose of suspect MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE lot U7211AA and suspect IPV (VERO) lot U7191AB. On an unknown date the patient developed a non-serious is reported as potential ae because of temperature excursion (product temperature excursion issue) (unknown latency) following the administration of MENINGOCOCCAL A-C-Y-W135 (D CONJ) VACCINE and (unknown latency) following the administration of IPV (VERO). It was reported "that she doubts an excursion actually happened and most likely might have been a glitch since it triggered after the data logger was plugged in and the back up data loggers did not record any temperatures out of range. See related case 01009162 for opened IPOL Additional Description of event Adverse events :Nurse is requesting stability information for unopened MENACTRA and opened IPOL after the data logger recorded 30.5 degrees F lasting for 14 minutes". Symptomtext

Client is 18 year old. She presented in clinic with her uncle, but came back into the treatment room by herself, since she is 18. Records were shown at registration that indicated client had had one dose of Covid-19 vaccine 9/13/21. When discussing with the client the vaccines she would need to enter school, staff commented that she had not completed the Covid vaccine series and asked if she would like her second dose. Client responded affirmatively, and another dose was given to the client. When client returned to her uncle in the waiting room and asked to stay for the 15 minute waiting period, her uncle asked her why she got another Covid shot when she'd had a second dose in October. The client said she didn't realize, and the uncle then produced a covid vaccination card with two vaccines recorded on it in the client's name: the one given, and another given 10/14/21 at a local pharmacy. This card was not presented at the time of registration, and the client admitted she had forgotten about the vaccination. Client waited 20 minutes after receiving her vaccinations, did not complain of or appear to experience any adverse effects. Client was instructed to call 911 or go to the ER if she felt any symptoms of anaphylaxis. Client's younger siblings returned to clinic the following week and uncle did not report any significant reaction/side effects in 18 year old niece. VAERS report made according to CDC guidance.
