Systemic: Dizziness / Lightheadness-Mild, Additional Details: Patient had two episodes of vasovagal feeling. 911 call was placed, Paramedics came, he felt better soon enough, his vitals were stable, he went home with his mother after eating and drinking Gatorade and water. No fall, no injuries either.

Symptomtext

The day after vaccination, patient started breaking out in a rash about 30 hours after vaccination. Patient had also been outside. She had no new foods, products, any other identifiable cause of rash. The rash is itchy, about 1 cm or less red spots with white halo. The rash was mainly on her arms and legs with sparing of her trunk and head although she had a few spots on her trunk. I am not at all convinced the rash is due to her shots although mom is. I did think I needed to report it due to timing.

Symptomtext

The meningococcal was given to patient twice within 2 weeks, once on 11/29/23 and once on 12/15/23. i asked pt several times when was the last time he got his meningitis shots, he said 2005, so i gave him the shot. After the shots, he asked me, when can he get this tetanus shot, so I look STC , I saw meningitis shot was given on the 29th of November of 2023. I told him I saw a shot on 11/29,/23 and i made him call the school and school said he did get it. He told he did not know he already got the meningitis. I made him wait for 45 mins and he was ok. He told me " I'll be fine". so i told him i will report it because he got it twice, and we will call to check on him after 2 weeks,. I also told him to call us if he as any problem I felt very very horrible, and I will look at stc for shots every time.

This case involves a 14 years old male patient who received their first dose of menacwy on 15oct2019 and booster dose of MENINGOCOCCAL A-C-Y-W135 (T CONJ) VACCINE [MENQUADFI] on 02aug2022 with no reported adverse event The patient's past medical treatment included MENACWY TT on 15-OCT-2019. The patient's past medical history, vaccination(s) and family history were not provided. Concomitant medications included HPV VACCINE On 02-Aug-2022, the patient received a 0.5 ml Booster dose of suspect MENINGOCOCCAL A-C-Y-W135 (T CONJ) VACCINE (lot U7249AA, expiry date: 11-Mar-2024) via intramuscular route in the right deltoid as immunization. On 02-AUG-2022 the patient developed a non-serious event "received their booster dose of menquadfi at 14 years old. they received their first dose of menacwy on 15-oct-2019 and the booster dose of menquadfi on 02-aug-2022 with no reported adverse event" (inappropriate schedule of product administration; latency: same day). No adverse reactions to report. There was no treatment. Action taken was not applicable. It was not reported if the patient received a corrective treatment for the event. Symptomtext

The Medical Assistant Reported me that Patient came to the clinic with an older brother who was 16 years old as well as his father on 08/02/202. Both children needed Well child checks and immunizations. Older male received MenQuadfi (secondo doses) and the affected patient only truly needed HPV but was also given MenQuadfi (second doses) Too. Menquadfi was given in the right deltoid. No redness or issues with administration of vaccine Once I notice that I have given the vaccine at the wrong age, the patient had already left the clinic. The MA went to speak with me ( vaccine coordinator). I explained I overlooked the date and knew the second dose is given at or after 16th birthday. I told her I will look into it and get back to her.then She went and spoke with Provider and explained the situation. She was told he would likely have to still get MenQuadfi on his 16th birthday to complete the series. Mother of the patient was notified on 08/09/2022 and was told to just keep an eye on her son. She denied any side effects or reaction. Mother was very understanding of the situation and understood that her son will still need the vaccine at age 16 to complete the series. Moving forward MA will have her co workers double check and review to be sure she has the correct immunization as well as the provider. The medical assistant is working before she administers the vaccines. She also will be doing additional training on immunizations to be more familiar with vaccines and the vaccines schedule. I called Sanofi Medical Information to report that incident : MenQuadfi is a vaccine indicated for active immunization that is approved for use in individuals 2 years of age and older. But as a booster The medical recommendation is to repeat the vaccine MenQoufut on Patient 16th birthday. no

Symptomtext

Patient woke up 10 days after vaccines with what appeared to be bleeding in the white part of the far corner of left eye. Patient already had appointment with his pediatrician. MD told parents "He probably scratched eye in his sleep." On the next day, patient woke up with the same symptom on the right eye. Mother took patient to see his eye doctor. Patient examined and cleared from having scratches/anything that could have caused this. Optometrist advised patient's mother to file an adverse report for vaccines received.

gave a 10 year old a Menquadfi booster dose with no reported adverse event; Initial information received on 16-May-2022 regarding an unsolicited valid non-serious case received from a other health professional. This case involves a 10 years old male patient gave a menquadfi booster dose with no reported adverse event while receiving vaccine MENINGOCOCCAL A-C-Y-W135 (T CONJ) VACCINE [MENQUADFI]. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included DIPHTHERIA VACCINE TOXOID, PERTUSSIS VACCINE ACELLULAR 3-COMPONENT, TETANUS VACCINE TOXOID (BOOSTRIX) for Prophylactic vaccination; and HPV VACCINE VLP RL1 4V (YEAST) (GARDASIL) for Prophylactic vaccination. On 15-Apr-2022, the patient received a dose of suspect MENINGOCOCCAL A-C-Y-W135 (T CONJ) VACCINE lot U7249AA (expiry date: 11-Mar-2024) via intramuscular route in unknown administration site for Immunization. On 15-APR-2022 (Latency: Same day) the patient developed a non-serious gave a 10 year old a menquadfi booster dose with no reported adverse event (product administered to patient of inappropriate age) following the administration of MENINGOCOCCAL A-C-Y-W135 (T CONJ) VACCINE. Action taken with MENINGOCOCCAL VACCINE (MENQUADFI) was not applicable.
