Fall Feeling abnormal Pallor Syncope Tooth fracture

Symptomtext

Patient received Meningococcal, HPV, and TdaP in clinic 07/26/2022. After receiving vaccines, she was standing at the front lobby with her Mother when she apparently had a syncopal episode. Mother attempted to catch her but was unable. The patient then fell from a standing position landed on her buttocks and then fell to her left side and chipped her left front incisor. She was in a sitting position in the floor as her Mother called out for help. The patient stated she "felt funny". Color was pale.The patient remained in the floor with her feet elevated as she leaned back on her Mother's chest for support. She was alert and oriented. Pink color returned to her face and she was talking. Broken tooth was evaluated by HD Dentist, Dr was instructed to f/u with her pediatric dentist. Patient was escorted to her vehicle with Mother. She will receive a F/U phone call from the HD 07/27/2022.

Actually, I am having quite a few reactions that are abnormal for me that I did not have before the shots. I went to the doctor on Friday for a yearly physical and she found a supraclavicular lymph node swelling on the right clavicle that I now have to get ultra-sounded on June 26th. The vaccines kicked my butt, especially the one given in the right arm. The night of and the couple of days after I have been sick with malaise, arm tingling and numbness, slight chest pain and exertion breathing issues/ stomach pain and having issues thermoregulating at night. I have had like almost cold flashes and then will get really hot and have been having issues sleeping. I had a slight fever the day of, but haven't had any since, but the arm numbness, stomach pain and swollen node are not abating and it is now Saturday. I feel like I need to report this and I don't know how to go about it. My doctor was really worried about the clavicle node until I told her I just had a vaccine a day and a half before but she said it is soft, moveable and tender which is a good sign it's vaccine related, but still needs me to be seen for it. I looked up side effects and adverse reactions on the web and these are all part of the B vaccine affects, but I'm wondering if I should be seen in ER, because some of them are kind of freaking me out obviously because you don't mess around with supraclavicular node issues.

Patient reported that arm is still swollen at appointment 5/9/23. Noted raised firm area measuring 9.5 x6 cm on left deltoid where Tdap injection given on 3/3/23. Patient reports arm started swelling that evening of injection on 3/3/23. Denies ever having any breathing difficulty, pain or itching at site.

Symptomtext

Pt c/o headache and dizziness post-vaccination. BP-144/91 P:95 O2 sat 100% at time adverse event began. No loss of conciousness witnessed. Juice given. Pt monitored closely by nursing staff for 30+ minutes. BP 117/70 P:66 prior to discharge. Pt stated feeling better other than a dull headache when discharged home with mother at 1225pm.

Pt reports since she went home after vaccination on Monday 7/25, redness/swelling/itching/pain at site of injection left deltoid. Measures 10 x 5.5 cm. 9/10 pain when she moves her arm. Not alleviated by ice pack and Tylenol. Pt reported fever yesterday morning-afternoon, alleviated by Tylenol.
