Symptomtext

Patient states the next day after receiving vaccinations that he broke out in a rash that itched. Patient appeared with a rash below elbow on each arm. Patient reported no allergies. Patient reported no redness or swelling at the site of the vaccinations. Patient was advised to follow up wit his primary care physician It is possible patient may have received previous vaccinations from another facility not recorded.

Symptomtext

Patient received tdap to left deltoid and mcv4 to right deltoid. Patient had no reaction after immunizations with no s/sx of distress or allergic reaction prior to leaving clinic after admin. Patients mother was talking to another mother in the lobby so they had stayed for a few minutes after. About an hour after immunization admin, patient and his mother returned to clinic. Patients right upper arm was visibly red, hot to touch and was firm around needle insertion site with edema present. Patients face was also visibly red and hot to touch. Patients color was otherwise normal in all other areas of the body, no cyanosis noted. Patient has no signs of shortness of breath or problems breathing, patient states he ?feels fine?. No swelling noted in mouth or opening of throat. Patient and his mother instructed to go to the nearest ER immediately for patient treatment. Patients mother stated she was taking patient to ?one of the hospitals?. At 1545 hospital contacted mother who was still at hospital with patient and he was receiving treatment for allergic reaction.

Symptomtext

Had vaccine at school administered by Health Department nurses on 03/31/2022 in AM. Woke up with rash on 04/02/2022 & took po Benadryl. No other s/s. Rash worse & large areas on shoulders, arms legs & especially on back of legs with no other s/s & went to Urgent Care on 04/03/2022 & given Steroid dose pack. Asof 04/06/2022 he is better. Mother is a nurse & says he has a lot of allergies.

Then became confused, like he was seeing things that weren't there -eg bananas looked like a dog

Pt was given the 0.2 ml of the pfizer vaccine that was not diluted/mixed before administering by a new medical assistant. The pt did not have any adverse reactions and the provider was notified before the pt left the office and was able to speak with the mom about the medication error.
