Pt and mother instructed to wait 15mins after vaccines. About 3 mins later, Patient fainted with sudden loss of consciousness with jaw tightness lasting 30 sec. Pt regained consciousness with neuro intact. A&O X4. No fall, no loss of bowel or bladder, no jerking movement or stiffness. Pt tolerated water and decline blood glucose checkup, and vomited after and stated, "he felt better". EMS was called, V/S was stable at b/p 116/68, hr -70, rr-18. Pt declined going to the ER. Pt and mother instructed to go to the ER with any seizure activities, loss of consciousness. Pt and mother verbalized understanding
