The child is a under care. The child immediately had a vasovagal reaction after injection of HPV vaccine. She did not loss consciousness . The FNP was called into the room and had the child lie down and drink Gatorade and eat a popsicle. Less than 5 minutes after she started to eat the popsicle, color had returned to the child's face. Within 10 minutes she stated she felt better. She was able to walk normally and move all 4 extremities well. The child denied pain. No swelling or redness noted at the injection site. No respiratory distress noted. After the 15 minute observation period, the child left the office. The child and staff who had accompanied child were instructed to call the clinic as needed,

Pt in for immunizations. Due today for Tdap, Men Quad, and HPV. VIS given to mother before administration. Tdap and Men Quad given in LD, Gardasil given in RD. After completion of administration of vaccines, pt had a near syncopal episode with singular body jerk. No loss of consciousness, no head injury. Pt A&O x 4 immediately following incident and no evidence of post-ictal state. No loss of bowel/bladder control and VS WNL. Pt follows commands and answers questions appropriately. Patient's mother called for assistance and patient was assessed by 3 clinic RNs and clinic supervisor discussed follow up care with mother of patient. Pt's mother verbalized understanding and ambulated with patient to lobby. Patient ambulatory with steady gait, no s/s of distress noted.

After orientating Mom on the vaccination process, the child is vaccinated. After vaccinating the minor, patient refers to feeling dizzy and begins to presents which suffers temporary loss of consciousness. She was offered the necessary care and first aid to stabilize the patient. After several minutes in observation the patient is stable, alert and oriented, Mom is recommended to visit a doctor.

After all vaccines were given (Tdap was given first on Right Arm, HPV9 second on Left arm and lastly Menquadfi on Left Arm) patient stated that eyes were blurry. Patient was instructed to lay down and once she was laying down her eyes rolled back and she started shaking. After a few seconds patient stopped shaking and that's when the provider came in. Patient waiting in the office for about 25-30 minutes.
