Patient here for seventh grade immunizations. Mom had chosen for daughter to receive TDaP, MCV4, and HPV. Twin brother also here, and received the same vaccinations prior to his sister. Patient did well after receiving the vaccinations. After a few minutes of sitting in the clinic room, patients eyes rolled back, her head fell back, body started shaking and her arms started contracting. EMS was called and nurse was able to arouse patient prior to ambulance arrival. Mom stated patient had not eaten anything yet and has never had a reaction after receiving vaccinations before. BP 110/62. Blood sugar and vitals checked by EMS. Patient was also given crackers and water and stated she felt ok after being checked out by
