Symptomtext

Parent and patient reported today (3/28/23) that for over a week post administration, the patient had weakness of the right upper extremity preventing her from being able to pick things up. They were unable to give an exact number of days this lasted. Patient stated the left arm hurt the first day and then the right arm began hurting and feeling weak. The day of administration, patient reports she had dizziness, 'nearly passed out at school,' and mild nausea. Mother stated she picked her up from school post PE after receiving the vaccinations and she did not look good. These findings were not reported to the office until today when asked if the patient had any reactions to the last dose. She reports being back to normal at this time.

Dizziness Presyncope Somnolence

Symptomtext

7:30 pm Patient with dizziness and presyncopal episode after vaccination. Per mother pt did not eat well and last ate over 7 hours ago. No prior hx of vaccine reaction. No hx of syncope. RNP walked in to pt laying on exam bed with post syncopal drowsiness. She was responsive to provider questions and able to follow commands. Vitals were stable. 102/68 , HR 86 laying down Pt observed office under RN care. Pt was given water and electrolyte drink without emesis. Repeat vitals 112/83 HR 83 sitting. Pt alert and orient x 3 Informed mother if pt unable to walk out independently recommend further observation and evaluation at downstairs ED. 8:05 pm Pt was able to stand and walk out of clinic with mother.

Symptomtext

Mother states 2 hours after injections, pt. started developing hives on her face, which later started on her arms as well. She did have a small amount of redness around the Tdap injection site. She was given Benadryl which helped. Her mother did call the child's pediatrician and was instructed to just watch her and to immediately take her to the ER if she developed breathing difficulty. She also developed a low-grade fever that was treated with Tylenol. She appeared better today, but still "felt a bit off" per her mother.

Patient presented to the clinic 2/2/23 with redness, swelling, and warmth around the injection site, Denied a fever. NP started patient on Bactrim DS 1 tab BID x 7 days. Patient came back on 2/6/23 for a recheck and arm looked a lot better with no swelling redness or warmth noted.

Dizziness Vomiting

Symptomtext

Patient laid down after vaccines as she was experiencing dizziness. Given water. Patient stayed laying down for 5 minutes, sat up for 5 minutes and then walked to leave the clinic; however, upon leaving the exam room patient complaining of more dizziness and vomited large amount of clear fluid. Patient placed in supine position and dizziness subsided. BP 102/60 P- 80 No respiratory distress. Patient remained for an additional 15 minutes, and then left alert and oriented x3 and walked out on her own. Patient monitored by RNP, RN and MA throughout course of this incident.

Symptomtext

Patient ID verified, no allergies per patient and mom. Patient was nervous and had not eaten since earlier in the day. Patient tolerated vaccines well with no reaction to vaccine sites. Patient became pale and clammy when standing up from his chair. Patient was laid down and monitored, BP 89/58, patient was alert and oriented. Monitored patient for 20 minutes BP 98/68 and patients color returned and stated he was feeling much better. Patient left Health Department with mom, alert and oriented and feeling better. On 04/18/2023 spoke with mom and mom states " they left Health Department and went to eat and patient was feeling well today.

This case involves a 16 years old male patient to whom inadvertently administered a dose of vaccine that had been exposed to temperatures lower than recommended for 3 hours and 15 minutes, reaching a minimum of 30 degrees f with no reported adverse event and undergone temperature excursion down to 30 degrees f for 3 hours and 15 minutes, due to a refrigerator malfunction with no reported adverse event involving vaccines influenza quadrival a-b vaccine [Fluzone Quadrivalent] and meningococcal a-c-y-w135 (t conj) vaccine [Menquadfi]. On 07-Feb-2023, the patient received an unknown dose of suspect influenza quadrival a-b vaccine (strength, formulation, lot number and expiry date was unknown) via intramuscular route in the left deltoid for prophylactic vaccination.Information for batch and expiry date was requested. It was reported regarding a non-serious event of undergone temperature excursion down to 30 degrees f for 3 hours and 15 minutes, due to a refrigerator malfunction with no reported adverse event (product storage error) (onset date: 07-Feb-2023, latency: same day) involving meningococcal a-c-y-w135 (t conj) VACCINE and influenza quadrival a-b vaccine. On 07-Feb-2023 the patient developed a non-serious event of inadvertently administered a dose of vaccine that had been exposed to temperatures lower than recommended for 3 hours and 15 minutes, reaching a minimum of 30 degrees f with no reported adverse event (poor quality product administered)(Latency: same day) following the administration of meningococcal a-c-y-w135 (t conj) vaccine and following the administration of influenza quadrival a-b vaccine.
