Symptomtext

Patient came into clinic as a walk-in triage. Patient had a swollen red area on the right deltoid due to receiving vaccines from clinic visit 06/05/23. The vaccines the patient received were 1st dose Men B and 2nd dose MCV4. Patient did confirm that MA did give VIS sheet to mother 06/05/23. Patient stated she had trouble breathing, itchinness and pain in arm, pain level is a 8/10. Patient stated that theses symptoms started around 7 pm 06/05/23. Asked patient did she take any medication, patient delined of taking any medication. Asked patient did she inform mother about the difficulty breathing. Patient stated mother was not at home. Asked patient has this reaction ever occurred when receiving a vaccine, patient stated yes when she received her 1st dose of MCV4. Asked patient what were the things she did to get the swollen red area to go away. She stated taking tylenol and a shower and it went away. Discussed walk-in triage with provider MD, per provider MD advised patient to take tylenol or ibufropen, benadryl and place a ice pack on area. Provider MD prescribed diphenhydrAMINE (Sominex) 25 mg 1 tablet every 8hr PRN. Provider MD also advised patient if she has SOB, wheezing, swelling of the lips/tongue or fainting, to go to the ED. MD advised patient if any further vaccine are needed, that she waits 30 mins for possible reaction from the vaccine. Nurse provided patient with VIS sheet in another language. Patient verbalized understanding.

Symptomtext

Patient was scared to get her vaccines mom informed us. Prior to vaccines Patient resisted to receive first vaccine and I was able to calm her down and she sat still to receive vaccines. After administartion Patient would hold her breath and mom and I would have to tell her to take some deep breaths and she would respond by talking or taking some breaths. Within a few minutes after vaccines I saw a change in her face color and asked her if she felt ok. She stated she needed to get sick. She laid on exam table and after 5 minutes felt better. BP was within normal limits. Mom stated she passed out when she got her ear pierced recently. I explained if she starts to feel not good then she needs to lay down or if she has anything life threatening go to ER. 7/24/23 10:00 PM - Mom reached out to me on my personal phone at 10:00 PM to call her. I called her and Cami vomited before she got in shower and almost passed out in shower. She had ate dinner at a resaurant and then headed home prior to this. Their BP cuff was not working. Cami was laying down in bed and felt better, but not 100%. She just wanted to go to sleep and the mom was concerned. I explained usually any fainting/vomiting/etc happens within 5-10 minutes after vaccines. I also explained in the 12 years I have been working at LPCHD I have not had any reports of anything like this hours after vaccine. Mom wanted to know if she should take her to hospital. I explained that anything considered life threatening we send individuals to the ER. In this case i cannot recommend not to go to ER, but if she feels concerned and she does not improve she can choose to take her to ER. Mom said she would watch her for an hour and decide. I also explained common symptoms of BP problems are dizziness, headache, not feeling well, etc. I would follow-up in morning with mom. 7-25-23 10:00 AM Mom states that Cami feels OK and slept all night. She will take her to doctor if it happens again.

Symptomtext

Patient reported lighted headedness, spotted vision, then shortly after lost of vision around 931pm Immediately after I called for Dr. and layed patient on back with feet elevated and obtained pulse of 49 and o2 of 100. Patient regained vision, drank water, continued to stay laying for 45 mins while watching vitals stay stable. Patient left clinic after provider discharged.

Patient given a Menquadfi when he was due for a MenB vaccine. He had a Menquadfi on 06/14/2023, when a booster if needed is not recommended for 3 years. Extra dose given, we have been unable to reach patient to confirm if he had any complications from this extra dose and to notify him of this error.
