Syncope

Symptomtext

Client was scheduled to receive vaccines at her school immunization clinic. Client had a syncopal episode lasting approximately 15 seconds following Tdap and MCV4 injections. She was given juice to drink and an ice pack to back of neck then felt much better and was ready to get the other vaccines. She had another syncopal episode after receiving the HPV and Flu vaccine injections, this episode also lasting only a few seconds. She drank more juice and was escorted to the school nurse's office where she rested and then was able to return to class.

Symptomtext

Administered meningococcal into R deltoid, Patient stated her r arm felt heavy. Administered Tdap into L deltoid, immediately after withdrawing the needle, hers eyes rolled upwards and she lost consciousness. Laid her down as much as possible, ( we were in a mobile vaccine van) she aroused after about 10 seconds. Her arms did stiffen while she was arousing. She was alert to person, place and time. She said," I don't feel good, I think I am going to throw up. " Gave her an emesis bag and she did vomit approx 120 ml of semi solid emesis. Her face was pale and clammy. BP was 92/60, heart rate was 100 and regular. Watched her for approx 20 minutes. she wanted to go home and mother was ok with her going home. advised the mother to take her home and let her lie down, give her some cool and sweet to drink and watch her for a couple of hours. instructed the mother to call 911 if any issues arise on the drive home. patient was still a little weak and was escorted to car by staff.

Syncope and possible seizure activity; after laying down and BP and pulse stabilized, no more seizure activity. Patient came up agitated and then calmed down.

Menquadfi and Bexsero were administered in the office at 1200 on 7/28. Father was present at time of injections and offered consent. Patient left the office and drove himself home shortly after the vaccines were given. 3:15pm: Pt. called his mom c/o feeling tired and not well, and was going to take a nap. 3:29pm: Pt. called his mom stating he had just passed out, felt strange and needed help. ~3:40pm: Mom arrived home and found pt., breathing, but not responding normally to verbal commands. Mom drove pt. to the nearest ER, about 10 minutes drive. During the drive, pt. experienced extreme neck pain, chills, headache and began convulsing. Upon arriving at ER, pt. was moved to trauma bay where he received Ativan, Bendaryl, and steroids. A CBC was drawn and WNL. Pt. remained in ER for 3 hours under observation and was discharged home that evening feeling well and 'normal." Pt. has no recall of these events after calling his mom at 3:15pm.

Symptomtext

Immediately following the MenQuadfi, pt. states her arm was hurting. She said moving the arm made the pain worse. She said she was okay to receive the second vaccine (her Adacel/Tdap), but while administering the vaccine, she stated she didn't feel good and lost consciousness. After about 10-15 seconds, pt. regained consciousness, but was pale and saying she didn't feel good. Pt. then vomited. Dr. went in to assess her and she was still pale and saying she didn't feel well. A few minutes later she vomited again and was given a zofran. 42 minutes into the incident, I checked her vitals again and they were WNL and pt was stating she felt much better and was ready to go home. She was reassessed by the Dr. and sent home stating arm pain was the only remaining symptom.

Received vaccines, approximately in an outdoor back to school event, 10 minutes later became dizzy, told mother he was going to faint. Mom eased client down, minor abrasion, posterior left calf, did not fall. Other event staff, came and asked to nursing assistance. This nurse responded. Client, alert and oriented, clammy, and pale, Per mom, Client had not gone to bed until 4:30 that morning and had not anything to eat or drink since awakening. Juice given. Client then became nauseated and vomited juice given. Unable to lay person down, outside, asphalt wet from recent rain. Client accompanied by two nurses inside church lounge. Client became unconscious, carried into lounge. Feet elevated, client immediately regained consciousness, alert, answered all questions appropriately. Additional juice given, color good, skin dry. After several minutes client feeling better, client sat, stood and walked with standby asst of two. Client walked to mom's car with standby asst of two. Mom advised to give client food with protein immediately and push fluids. Advised to have client rest with feet up for a few hours.

1 week after receiving MCV, patient started having outbursts of aggression & behavioral problems. She is hitting, biting, throwing & breaking objects at home. This has continued to the present day though there has been some improvement with time. Patient is nonverbal but previously could be redirected & parents cannot redirect through these episodes. She was biting occasionally before this but less frequent.

8/8/23 4:02pm: Mother states patient has mild L arm stiffness after the vaccine in L deltoid. Advised to move the arm a bit, give it 5 mins, see if she feels better after that. 8/8/23 4:10pm: Mother states pt still has L shoulder pain, feeling of fullness, in the L shoulder, would like to see a dr. Advised Dr, he will look in on her. Pt breathing normally, no visible signs of acute pain or allergic reaction. Provider saw pt. today, she was then released home with mom. 8/9/23 3:06pm Nurse called patient mom to discuss labwork results and mother mentioned that patient still has pain in the left shoulder and neck. She has limited range of motion in her left shoulder. Mom continuing to give motrin and apply ice to the area. It has been about 24 hours since she got her vaccine. 8/10/23 12:09 pm Dr. called and talked with mother, pt. is still having shoulder pain but is getting relief from ibuprofen and ice, now heat. I advised mother that I think it is best I send a VAERS report; I offered to schedule an appointment with neurology, mother would prefer to wait and is thinking since it is shoulder pain maybe a referral to PT is more appropriate. I advised her maybe ortho but will hold all referrals for now, pt. has a visit with Dr. next week and the plan is to follow up then. Mother will call sooner if pt. has worsening symptoms. I will report again the need to review with nursing staff proper vaccine administration protocol. [08/10/23 12:09pm]

Symptomtext

Patient reported that within 5 minutes of getting the injection, she got a sudden headache that lasted 10 minutes. About 10-20 minutes after the headache, she developed bilateral ringing in her ears and ear pain. This also lasted about 10 minutes and then resolved. She continues to have a mild/trace headache 2 hours after injection and feels tired. Via phone call, Advised patient to hydrate, rest, and monitor symptoms. Advised to take Tylenol PRN for continued headache. Advised to call back if symptoms persist after 24 hours

Symptomtext

pale, shaky, feeling weak initially, then sporadic twitching began, gait unsteady. Increased wheezing noted, albuterol neb given, BP slightly elevated compared to when pt arrived. Twitching continued but color improved, pt feeling better. She went home however twitching continued so went to ED next morning. Eval included labs, MRI +/- contrast of brain and spine. No acute abnormalities. She's still in hospital being observed, twitching decreasing. Wheezing ongoing so getting q 2 hr albuterol NO rashes,, vomiting or diarrhea, no swallowing difficulties.

On 05/03/2023 the notified Dr. that she draw and give the wrong vaccine, pt. was in a rush and the MA prepared meningococal vaccine opposed to the Prevnar 13. Pt. was called by both Drs. to explain the situation and assure her that no harm or risk would come o her child.
