Hypotension Syncope

Symptomtext

At 10:13 am, patient experienced syncope and hypotension. No loss of consciousness. Patient laid down with feet elevated and blood pressure taken. Cool compress applied to forehead. Offered juice. Monitored blood pressure which returned to normal range at 10:20 am. Patient consumed 8 oz juice and reported she felt better. Symptoms improved and patient monitored for an additional 15 minutes. Advised to seek emergency care if symptoms return again. Patient left clinic at 10:40 am with adult guardian.

Symptomtext

11am Patient passed out in the chair after administering 2nd vaccine (HPV). Patient started leaning back, head tilted and started having seizure like attack. Patient lowered unto the floor with the help of his father and other staff. Patient seemed to be having grand mal like seizures which lasted about a 1.5 minutes. Patient was also groaning while twitching. Vitals taken BP 132/76, pulse 64, RR 20 Temp 98.6. 11:05 BP 115/67 pul 64. Patient was able to communicated and complained to abdominal pains. Stated he felt like vomiting. Patient father stated that he had fainting episodes when he gets an injection. Patient denies any history epilepsy or neuro disorders. 11:15 BP 121/65, pul 62. RR 20 patient c/o of feeling tired and wanted to rest. Repositioned on the floor. Patient reported he had similar faint episode in his Biology class2 yrs ago but could not recall what happen. He however stated that his friends told me that "his head tilted backwards when he passed out 12:00pm Patient c/o of nausea. Vomited and reported feeling better. Father encouraged to take patient to ER/ or doctors office for further medical evaluation Patient leaves with his mother and is here on vacation to see his father.

AFTER THE 2 VACCINES WERE ADMINISTERED, THE PATIENT WAS FINE. MOM, THE PATIENT AND MY SELF WERE WALKING OUT OF THE ROOM. I WAS IN FRONT AND MOM AND PT WERE BEHIND ME. I HEARD MOM SAY HELP. I TURNED AROUND AND SAW PT FALLING TO THE GROUND AND MOM WAS HOLDING HER ON ONE SIDE AND THEN A MEDICAL STUDENT CAME OUT OF THE OFFICE. DR. AND I WERE HOLDING HER ON BOTH SIDES BUT PT STARTED TO PASS OUT BEFORE WE COULD GET HER TO THE EXAM ROOM. WE IMMEDIATLEY LAYED HER DOWN ON THE FLOOR UNTIL WE COULD GET HER SETTLED. DR. CAME OUT OF THE OFFICE, TOO. PT "WAS OUT COLD" FOR AT LEAST 5 MIN. DR. WAS ABLE TO AROUSE HER BUT PT DIDN'T KNOW WHERE SHE WAS OR WHAT HAPPENEND. DR. HAD MOM MOVE IN SIGHT OF PT TO CALM HER. AT THIS TIME PT HR WAS 120s, PALE, NYSTAGMUS, UNABLE TO FOCUS, STARTED TO BECOME NAUSEOUS. WITH HELP, WE WERE ABLE TO MOVE PT INTO THE EXAM ROOM ON THE TABLE. PT WAS COMPLAINING SHE WAS COLD SO WE COVERED HER ON THE TABLE. SHE HAD HER HEAD AT 45 DEGREE ANGLE. SHE BEGAN TO COMPLAIN HER STOMACH HURT AND THEN SAID HAND WERE TINGLING AND SAID SHE COULD MOVE HER HANDS. THEN SHE GOT WARM. ONLY TJE HANDS BECAME STIFF (DYSTONIC). SHE WAS DIAPHORECTIC AND SHAKING AT THIS TIME BP 79/42 HR 80s. WE HAD INJECTABLE DRUGS (COMPAZINE AND OTHERS) BUT PT WAS GETTING ANXIOUS ABOUT THE NEEDLES SO DR. TOOK A BENEDRYL 25MG CAPSULE AND POKED HOLES IN THE CAPSULE AND MADE HER TAKE IT THAT WAY. IT DID WORK WITHIN SECONDSTO RELEAVE THE DIAPHORECTIC. WE STARTED GIVING HER WATER AND PRETZELS. WE FOUND OUT SHE ONLY ATE WATERMELLON FOR BREAKFAST.SHE CONTINUSED TO RESTFOR ANOTHER 20-30 MIN WITH EATING. FELT BETTER, COLOR WAS COMING BACK TO HER AND FELT STRONGER SO SHE DECIDED TO GET UP AND TOOK 2 STEPS AND WAS CARRIED BACK TO THE TABLE. PT THEN WAS FREEZING AND THEN SHE FELL ASLEEP. DR. ADVISED MOM PT NEEDED TO GO TO THE HOSPITAL. DAD ARRIVED AT THIS TIME AND MOM AND DAD FELT AN AMBULANCE WOULD BE BEST. I CALLED THE AMBULANCE GAVE THEM THE HISTORY AND THEY ARRIVED APPROX. 15MIN LATER. THEY ASSESSED HER- GLUCOSE 130 ON METER, + ORTHOSTATIC HYPOTENSION AND AGREED SHE SHOULD GO TO HOSPITAL ED. THE TOTAL TIME IN THE OFFICE 12:15PM-2:15PM WE DID CHECK THE VACCINATION AREA AND THERE WAS NO REACTION VISIBLE.

Symptomtext

Patient experienced near syncope 10 minutes after administration of vaccine. Patient transported back to exam room where attended by physician. Vitals were monitored. Pt was initially bradycardic with low blood pressure. Pt monitor closely for 30 minutes. Alert and oriented throughout. Mother present throughout. Vitals stable. Patient eating and drinking refreshments in office provided by staff. Walked out of clinic without any issues.

Symptomtext

Post meningococcal and MMR vaccines, pt verbalized dizziness and nausea, then proceeded to vomit twice. Observed pt looking pale and diaphoretic. VS initiated, legs elevated on chair, ice packs, water, and lollipop provided. Pt was reassessed after 10 minutes, pt still appeared pale and diaphoretic, pt was carefully moved to the sofa with another RN. RN Observed VS and color normalized after lying down. Pt was moved to observation for continue monitoring. At 2:50pm, Pt verbalized symptoms fully resolved, pt left observation in stable conditions with parent. Education and teaching provided to pt and parent.

Dizziness Pallor

Symptomtext

On 9/26/23 at approximately 1556 a nurse alarm was called. At approximately 1557, 2 PHNs arrive on scene with patient and Mother of patient, 13 year old female was found conscious, pale, and sitting on floor of lobby accompanied by mother and patient's sister. Per patient's mother and patient shortly after receiving vaccines patient felt dizzy upon exiting clinic and was assisted to the floor by patient sister prior to nurse arrival. Patient's mom reported patient waiting 10 minutes after vaccination for observation period prior to nurse arrival. Patient was alert to name, location, and situation. Patient denied any current medications or chronic conditions. Patient's sister and patient's mom denied patient fainting. Patient's mother reported history of vaccine anxiety and dizziness with previous vaccinations. Patient's vital signs were O2 99%, heart rate 66 bpm, respirations 16, blood pressure was 86/60. Pt. had water bottle with her and PHN encouraged her to drink and she drank. Pt. reported feeling better, denied pain. EMS offered to be called, patient's mother declined services. Patient and patients mom encouraged to stay to further observation, patient's mom declined. Patient's mother educated to monitor pt. closely for signs and symptoms of concussion and seek medical attention. Patient's mother provided concussion fact sheet for patients and educated patient and patient mom to eat and hydrate prior to future vaccinations. Patient's mother verbalized understanding and had no further questions. At approximately 1610 patient stood without assistance and exited the building with a steady gait accompanied by patient's mother and patient sister. Patient received HPV and MenACWY vaccines.

Symptomtext

Patient received Jynneos, MenQuadfi, and Gardasil on July 21, 2023. MenQuadfi and Gardasil was administered both on right deltoid. Patient denies on any reaction for Menquadfi and Gardasil. Jynneos was administered on left arm, subcutaneous route. After two days, patient noticed a itchiness and big red rash on the left arm, on which the Jynneos was given. Patient noticed the reaction while at amusement park around 0900, patient continued about his day and when shower on Jul 24 after showering the area had increased. The rash measurement is 10cm. Then proceeded to work and was seen my his primary care provider which directed him take a Benadryl to go back to the clinic to get checked.

Symptomtext

Patient seen in ER one hour after symptoms began for flushed cheeks, lightheaded, throat tightening, scratchy throat, swollen neck, rash, and hives across chest and face. 0.3mg of epinephrine given for treatment of anaphylaxis. Patient also ordered solu-medrol, pepcid, and benadryl for further management. Patient observed for 4 hours after epinephrine administration and was then discharged home with EpiPen.

Symptomtext

Developed whole body hives 4 hrs after the vaccine. No wheezing or breathing problems. Parents are Benadryl and the hives resolved by the next day. This is his first time getting the MenQuadFi, his first meningococcal vaccine was Menactra in 2018.

Patient here for immunization for school. Grandmother here with patient. During the immunization risk assessment, patient reports "I have history of seizures that started in April and has seizures very 2-3 weeks. I am taking seizure medication that the doctor double up on and it has helped a lot." Denies any other problems in agreement with grandmother. Needs meningococcal ACWY. Meningococcal vaccine information statement given to grandmother with risks discussed. Meningococcal vaccine given in right deltoid. Patient tolerated well. Awake, alert and tolerated well. Patient escorted to discharge desk at which point she became weak and was transferred to chair. Patient talking but reporting "feeling weird". Patient warm and clammy. 1600: BP 95/55 pulse 69 Resp 18. Patient sitting in chair with head resting on back of chair Patient sipping sprite. RN at chair side. 1615: 101/62 pulse 60 Resp 18 Patient awake and alert with color returning to lips and face. 1625 Patient sitting up in chair BP 110/65 pulse 56 resp 18. Patient states she feels able to ambulate to car with minimal assistance escorted by nurse and placed in vehicle sitting up in seat. Awake and alert. Grandmother at side. RN Supervisor notified. Grandmother and patient comfortable leaving clinic. Will follow up with patient. -------RN Notified that grandmother and patient are back in parking lot. Grandmother states patient is not feeling good again. BP 105/55 Pulse 65 O2Sat 100% Resp 18. Patient looked weak, slumped, oriented x 3, diaphoretic, and talking. 1641: EMS called. BP 1645: 98/59 pulse 68, pulse ox 100% continuous monitor per Nurse. P: EMS arrival 1645. EMS to assess and determine further need. Accompanied by RNs. EMS monitored and spoke with legal guardian by phone. Mother declined transport by EMS. Mother Chooses to meet patient by private vehicle. Patient transferred to vehicle by EMS ambulating, alert and oriented x 3. Care released to grandmother. Will follow up with patient. ----RN

nurse practitioner found the patient unresponsive in the passenger's seat; right after the injection, the patient leaned forward and looked flushed; mom said she does get anxiety with injections; pale; clammy; flaccid; diaphoretic; became limp with no radial pulse\pulse rechecked and weak radial pulse was noted; Initial information received on 08-Aug-2023 regarding an unsolicited valid serious case received from a other health professional. This case involves a 13 years old female patient and nurse practitioner administered the Meningococcal A-C-Y-W135 (T Conj) Vaccine [Menquadfi] after that found the patient unresponsive in the passenger's seat, right after the injection, the patient leaned forward and looked flushed, mom said she does get anxiety with injections, pale, clammy, flaccid, diaphoretic and became limp with no radial pulse\pulse rechecked and weak radial pulse was noted. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. On an unknown date, the patient received a 0.5 ml dose of suspect Meningococcal A-C-Y-W135 (T Conj) Vaccine solution for injection (frequency: once, strength: standard, expiry date: 27-Jun-2025 and lot U7643AA) via intramuscular route in the left deltoid for Immunization. On an unknown date the patient found the patient unresponsive in the passenger's seat (unresponsive to stimuli) (unknown latency). This event was assessed as medically significant. On an unknown date the patient leaned forward and looked flushed (flushing), she does get anxiety with injections (anxiety), pale (pallor), clammy (cold sweat), flaccid (hypotonia), diaphoretic (hyperhidrosis) and became limp with no radial pulse\pulse rechecked and weak radial pulse was noted (radial pulse decreased) (unknown latency). Relevant laboratory test results included: Radial pulse - On an unknown date: [pulse rechecked and weak radial pulse was noted no radial pulse] Action taken: not applicable. A sternal rub was corrective treatment was received for the event (unresponsive to stimuli). It was not reported if the patient received a corrective treatment for the events.
