After administering last immunization, Tdap, Meningococcal, HPV and Flu shot, approx. 1 minute later patient convulsed and made snorting noise lasting approx. 15sec. Grandmother states patients father does the same thing after injections. Grandmother states patient does not have history of seizures or family history. Patients skin cool and clammy and could not keep eyes open for 15 seconds. Patient aware of surroundings, but drowsy for 3 minutes. Normal respirations, pulse. Offered to call ambulance transfer to ED. Declined by grandmother. Advised grandmother to seek further medical attention if needed. Assisted patient to personal vehicle in wheel chair, Patient stable and alert when leaving health unit. Grandmother states she was taking patient to ED. 1600 TC from patient grandmother stating patient was seen in ER and released, ER stated patient had vasovagal reaction from immunizations.

Patient received 3 IM injections during routine immunization visit, and shortly after 3rd injection had approx 10 second unresponsive "fainting spell", came to, but continued to feel unwell, was pale and sweaty, and complained of legs feeling "weird" and shaky. Had patient lay on floor and EMS was called to assess patient. After patient condition improved after approx 10 minutes, patient was cleared to leave clinic with mother.

Patient lost consciousness 5 minutes after shots given. Patient was positioned supine and comfort measures done. Vital signs within normal limits. No other symptoms at this time. patient resumed consciousness after approximately 30 seconds. 15 minutes later after slowly raising head of exam table, patient walked to chair and lost consciousness at chair, with head hyperextended and eyes oriented to left upper field with patient having snoring respirations. no tonic movements of limbs noted by myself, the reporting R.N. Gained consciousness at approximately 20 seconds and subsequently vomited a moderate amount of yellow fluid. Patient talking and answering questions. EMS called and report given to crew. EMS then assumed care. patient c/o dizziness to EMS personnel.

After receiving all vaccinations, patient was asked if okay to stand up. He stood up and walked about 5ft, standing there after 3-4 minutes, patient fell forward in a stiff/muscle tension position. Resulting in falling on his face/head. Patient did not show any symptoms before fall. After the fall, both mom and I turned him around, laying on his back. Provider did assist with helping patient gain conscious. We then noticed patient was bleeding from his mouth. We gave mom gauze for some pressure on childs lip. As then, we noticed more blood coming from his mouth and a missing from tooth. Patient also had an open wound underneath his chin. As patient was trying to gain conscious, there was some uncommon body movement and yelling come from him. As we all patiently helped with assisting our provider to make patient feel ok. He slowly responded with answering he was ok. Then paramedics where called by this time. As patient was still laying on his back, mother was not letting his head from hitting the floor. Blood pressure was taken a few times while patient was on the floor. Reading was taken around 11:20am. 1st 100/55 and 2nd reading 105/71, pulse 82, Sat 100%. Paramedics responded around 11:25, Care was then handed over to paramedics.

Patient standing up in hallway during checkout and stated that she didn't feel good. She leaned over on her mother and passed out, she was assisted by her mother and the nurse to the floor. She immediately regained consciousness and was assisted into a chair for monitoring. She reported nausea and began dry heaving. She had not eaten anything prior to coming into clinic. Vitals signs assessed for 30 minutes, felt better and released home with mom.

Patient received Menquadfi vaccine at approximately 9:35am. Vaccine received well and patient voiced relief she had gotten it over with and it was not as bad as she thought it would be. She walked out of clinic room and down hall where record was given without difficulty or complaint. Once patient in waiting room with stepmom, they stopped to get a drink out of Coke machine and patient stated to stepmom she was feeling dizzy. Stepmom states before she can ask her any further questions or turn around from getting drink from machine, patient falls to the floor hitting her as she was faced away from her. Staff was seated in clerical area and heard fall, looked up and noticed patient on floor. Staff had clerk call for help. All of this occurring at 9:40am. When nurses arrive out to lobby, patient is lying face down on floor unresponsive initially but breathing. Patient was gently log rolled onto back. First vitals at 9:41am, BP 102/56. Patient pale and bruising noted to left side of face as well as bleeding noted to left upper and lower lip. Patient slowly began to open eyes. BP at 0946 110/60. Slowly raised up to a 45 degree position but rested against nurse, being unable to hold herself up. Patient did become more responsive and verbal. Asked patient what she had for breakfast and quietly responded that she had cereal and the kind of cereal she had. Pulse at 0951 -76 slightly weak but regular. BP at 0952 106/58. Continued to support patient in seated position and patient was verbal when asked questions but kept eyes only slightly opened and closed most of the time. 0958 called patient's pcp and spoke with staff giving all details of episode thus far. He suggests to have patient evaluated at ER since patient did hit her head. Stepmom present and agrees this is what she wishes as well. BP at 0956 122/70 and pulse strong and regular. Patient remains verbal, but only when spoken to, and supported to sit by nurse. Patient's left leg began to shake and patient verbalizes she felt she was shaking all over as well. Ambulance notified of need for transport for further evaluation at 1001 and arrives by 1006. Verbal report and form copy given to ambulance personnel. Patient transported via ambulance at that time for further evaluation.

Mother stated that her son was having a reaction to the vaccines administered on Tuesday 08-01-2023. She stated that his arm was red, bruised, swollen around injection site and was hard around the " hole ". He also had a couple of bubbles at injection sites. Patient was also feverish. Mother stated that she administered ibuprofen and was putting cool compresses around sites. I suggested to her that she could take her son to his primary care provider if she felt that it was necessary and for further treatment. Educated regarding VIS forms that she received at time of vaccine administration and she verified that she did read them and noted the side effects listed.

After all 3 shots were given and bandaids were being applied the patient turned white and got light headed. I grabbed him and laid him down on the table. Dr came in and checked on him. We had patient lay there for awhile then he sat up for awhile on the table. When he felt better he walked to the chair in the room and sat down for the rest of the 20 minutes. Patient was able to walk out on his own with dad.

patient inadvertently received a dose of Menquadfi, she was intended to receive MMR/Varicella with no reported adverse event; Initial information was received on 28-Aug-2023 regarding an unsolicited valid non-serious case received from a other health professional. This case involves 5 years old female patient who reported that inadvertently received a dose of Meningococcal A-C-Y-W135 (T CONJ) vaccine [Menquadfi], she was intended to receive MMR/VARICELLA with no reported adverse event. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included Hepatitis A vaccine and diphtheria vaccine toxoid, pertussis vaccine acellular, polio vaccine inact, tetanus vaccine toxoid (TDAP IPV) for Prophylactic vaccination. On 21-Aug-2023, the patient received a dose 1 of 0.5 ml of suspect meningococcal A-C-Y-W135 (T CONJ) vaccine (Solution for injection) (lot U7643AB, Expiry date : 27-Jun-2025, strength was unknown) via intramuscular route in the right arm for Immunization and inadvertently received a dose of Menquadfi, she was intended to receive MMR/VARICELLA with no reported adverse event (wrong product administered) (latency : same day). It was reported that "Caller reporting the patient has not received prior doses of meningococcal vaccinations on their records, but she didn't think it was approved for a 5-year-old to receive. This situation is reported as a medication error due to administering vaccination in error." No lab data reported. Action taken was not applicable. At time of reporting, the outcome was Unknown for the event patient inadvertently received a dose of Menquadfi, she was intended to receive MMR/VARICELLA with no reported adverse event. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the guidelines. This case involves a 11 years old male patient for whom a vial of meningococcal A-C-Y-W135 (T CONJ) Vaccine [Menquadfi] was prepared for use on 01aug2023 but the patient did not come and the product was administered on 02aug2023 to another patient (with no reported adverse event) while receiving vaccine The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included HPV vaccine VLP RL1 4V (Yeast) (Gardasil) and diphtheria vaccine toxoid, pertussis vaccine acellular 3-component, tetanus vaccine toxoid (Boostrix) for Prophylactic vaccination. On 02-Aug-2023, the patient received 0.5 ml (dose 1) of suspect meningococcal A-C-Y-W135 (T CONJ) Vaccine, Solution for injection lot U7643AB via intramuscular route in the left deltoid for immunization,a vial was prepared for patient use on 01aug2023 but the patient did not come and the product was administered on 02aug2023 to another patient (with no reported adverse event) (poor quality product administered) (latency same day). Action taken: not applicable. At time of reporting, the outcome was Unknown for the event poor quality product administered. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. Nurse was given immunization encounter without a list of previous vaccines received by client and gave the requested meningococcal vaccine. After the vaccine was given, the clerk gave the immunization print out to the nurse and it was discovered the client had the vaccine 6/2022 and did not need the vaccine given today. No reaction to vaccine noted.
