Mom called into clinic on 12/14/2023 to report that patient had developed a red, blotchy, lumpy, itchy rash over the injection site, approximately 3-4 inches across. The area over the injection site was painful to palpation and mom reported patient was complaining of itching at the injection site where the rash was present. The rash was blanchable, not bleeding or oozing, did not look fluid filled. Parents voiced concern that the injection site was too lateral and there was concern the injection was administered into an area of the thigh that was more fatty tissue than muscle. MD phoned and spoke to mom that same day of report at 8:48pm Mom reported at that time the patient's condition was about the same as when she messaged in and talked to the RN earlier in the evening. No worse. Redness had not exceeded the line she drew around the affected area. Did give cetirizine 5mg and ibuprofen. Patient was comfortable at time of communication with mother. Mother also relayed to MD that the redness wasn't there Tuesday or Wednesday but showed up Thursday. Tender and "lumpy" at injection site. No streaking. No fever. Reviewed plan for follow up evaluation at clinic 12/15, possible antibiotics, possible antihistamines. May need repeat vaccination. Yes, ok to split up the shot into dtap and ipv as separate shots if repeat coverage is indicated. MD Recommend to ER overnight if fever, streaking, other concern. MD Reviewed our ofc plan for addressing proper vaccination administration with staff. Mom expressed understanding and agreement.

Possibly viral pharyngitis. Patient was able to swallow at time of visit on 12/9/23. Appeared non-toxic. Instructed to return if not better in 7 days, looking more ill, unable to swallow. Symptomatic therapy to include ibuprofen or acetaminophen as desired for pain or fever.

Patient received a number of vaccinations as a part of her routine 4 year old well visit on 11/29/2023. Her mother called on 12/1/2023 stating the site on the patient's right thigh, where she had received Quadracel and influenza vaccinations was red and swollen and was increased from the day prior. Patient was started on oral antibiotics (clindamycin) and given an appointment to be seen in clinic on 12/1/2023, the next available appointment. She was evaulated on 12/1/2023 in our clinic with improvement of the erythema and swelling.

Patient complained of leg pain after vaccines but seemed appropriate given context - the next day parent noticed some redness and swelling of the left leg, which became much worse at the end of the day 11/09/2023. Swelling reported entire thigh to knee, parents were advised to take patient to the ER for follow up. He was administered benadryl and discharged home for follow up in office the next day.

developed a 1.5cm blister at the site of injection; Initial information was received on 22-Sep-2023 regarding an unsolicited valid non-serious case received from a other health professional. This case involves 4 years old and unknown gender patient who developed a 1.5cm blister at the site of injection after receiving diphtheria-15/tetanus/5 hybrid ac pertussis/ipv (mrc5) vaccine [Quadracel]. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. On an unknown date, the patient received a unknown dose of suspect diphtheria-15/tetanus/5 hybrid ac pertussis/ipv (mrc5) vaccine (Suspension for injection) (lot U7656AA, Expiry date : 08-Mar-2025,strength was unknown) via unknown route in unknown administration site for Prophylactic vaccination (Immunization). On an unknown date the patient developed a 1.5cm blister at the site of injection (vaccination site vesicles) (latency : 1 day) following the administration of diphtheria-15/tetanus/5 hybrid ac pertussis/ipv (mrc5) vaccine. Action taken with DIPHTHERIA, TETANUS, AC PERTUSSIS and ipv VACCINE (Quadracel) was not applicable. It was not reported if the patient received a corrective treatment for the event (developed a 1.5cm blister at the site of injection). On 08/09/23 Pt. was seen in our office for her routine four year old well exam and received Dtap-IPV IM on her right thigh, and MMRV SC on her left thigh. Per mother of patient, patient had reported that her right thigh felt very itchy after the immunization. On 08/10/23, mother of patient reported that patient woke up with a circular rash approximately the size of a quarter on her right thigh; location where the Dtap-IPV had been administered. Later that afternoon, the rash had increased in size and patient was brought to Urgent Care where the attending provider documented an erythematous rash on right upper thigh, warm to the touch and indurated measuring approximately 14 cm x 10 cm. Patient was prescribed Cephalexin oral antibiotics for 7 days and instructed to take Benadryl every 8hrs. Rash circumference was marked by attending and mother of patient was instructed to administer Cephalexin if rash increased the following day. On 08/11/23 pt was brought in by mother to be assessed by walk-in provider as rash had continued to spread on right thigh, covering right thing in it?s majority. Pt.'s mother reports to have administered the cephalexin antibiotic on 08/12/2023 and that the rash began to improve at that time. As of 08/14/2023, I spoke with pt.'s mother who stated that the rash had now resolved after completion of the seven day course of antibiotics.

About 45 min post vaccines, face was flushed and complained of headache. He then started screaming that his hands itched. He angrily threw his body on the ground screaming and grabbing at his face and neck. He then seemed to ?.fall asleep?. His body was very limp but he never stopped breathing. We talked to him consistently. He would open his eyes with very slow speech responses for maybe 10 Seconds at a time and then ?sleep? again. I called 911 but they couldn?t understand me and I didn?t want to waste time. His eye got red and pupils shrunk. I put him in the water park spray and kept having him talk to me. He continually went in and out of ?sleep?. I placed him in car and kept talking to him and commanding him to drink cold sips of water. Drove 5 minutes and carried him limp in and out of ?sleep? into the clinic. He said he was hot. Then he suddenly got very cold with chills. Everyone took vitals. I asked for something to give him many times, and he deeply fell asleep starting to snore. 3 providers assessed him. 911 was called and finally About 15 min later, epinephrine was given. Immediately after the epinephrine He ?woke up? and began taking deep big breaths and came to. Over the next 10 minutes he became himself again. We were transported by ambulance to the emergency room where they did vitals and an ekg and heart ultrasound. No abnormalities noted and we were discharged. 24 hours later I have documented pictures of his left arm with a large red ring. He stated it was very ?itchy?. He has been himself since.

Vaccines were held, Vaccine companies contacted by staff and on 9/18/23 was told by her these vaccines were safe to give including the DTAP/IVP. But then on 9/29/23 staff with office contacted me again at our office and said after 2nd review, the DTAP/IPV might be INEFFECTIVE after all and should contact anyone who had dose of this to notify them possibly INEFFECTIVE, but NOT HARMFUL. and that they should come back after a month to have vaccine repeated. Called patient's mother and notified of this information. She chose to bring patient in for appt scheduled 11/6/23 at 8:30am to have this vaccine repeated, so appt still pending. She did not sound upset on the phone.

On 8/3/23 patient was brought to our office by a relative for a routine well child examination & immunization update. A health appraisal was requested & the child's immunization record was printed at approximately 1:30PM 8/3/23 from database, to attach to the form. I explained to the relative how to fill the form, we discussed the immunizations that were due indicated by the printed database record, how they would be administered, aftercare/possible side effects, & that I would hand write in the immunizations received on the printed record, which I initialed, signed, dated & stamped after the appointment. The patient was given Proquad (administered subcutaneously right thigh top) & Quadracel (administered intramuscular in the left vast lat) & tolerated it well. Once the visit was completed the relative & patient left the office. Around 2:30PM, the patient?s mother called from work & spoke with our receptionist & Dr., stating that she had taken the child to the Health Department that morning to have his immunizations. Once I was informed of this, I immediately called the immunization program coordinator at Health Department to see what needed to be done & what the parent needed to watch for. She stated that the child should be fine, but to tell the mom to watch for fever, alternate Tylenol & Motrin, & if fever was not controlled by this method, she should take the child to the ER, & that I needed to make a VAERS report. Follow-up was completed with the patient 2 weeks later (08/17/23) & no adverse side effects were reported.

This case involves a 15 month old female patient who received Diphtheria-15/Tetanus/5 Hybrid AC pertussis/IPV (MRC5) vaccine [Quadracel] instead of the intended Diphtheria/Tetanus/5 Hybrid AC pertussis/IPV(MRC5)/HIB(PRP/T) Vaccine [Pentacel] with no reported adverse event. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included Pneumococcal Vaccine CONJ 15V (CRM197) (Vaxneuvance) in the right thigh for Prophylactic vaccination. On 25-Jul-2023, the 15 month old patient received a dose of 0.5 ml (Frequency: once) of standard strength dose 4 of suspect Diphtheria-15/Tetanus/5 Hybrid AC pertussis/IPV (MRC5) vaccine [Quadracel] suspension for injection (lot: U7656AA, Expiry date: 08-Mar-2025) via intramuscular route in the left thigh for prophylactic vaccination (Immunisation) instead of the intended Diphtheria/Tetanus/5 Hybrid AC pertussis/IPV(MRC5)/HIB(PRP/T) Vaccine [Pentacel] with no reported adverse event (product administered to patient of inappropriate age) (wrong product administered) (latency: same day). Action taken: not applicable. At time of reporting, the outcome was Unknown for both the events. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices.
