Dyskinesia Immediate post-injection reaction Syncope

Symptomtext

Injection was administered at around 11:30 AM,. Immediately after the injection the patient started to faint and showed some hand movements. The pharmacist and technician helped the patient to the floor, where the patient continued to faint and had more hand and leg movements. after a short period the patient started responding to questions and wanted to sit up. Once the patient moved from the floor the pharmacist noted a wet spot under where she was laying down and concluded that patient may have voided her bladder. The patient was given some water to sip and asked for her sister to be brought into the pharmacy. The patients caregiver gave her orange juice and the pharmacist called EMS and waited for paramedics. Upon arrival the paramedics evaluated the patient and the patient opted to leave for her home with her sister.

Symptomtext

Right arm was warm to touch, raised red rash measuring 8.5 in L x 8in W, tenderness present . Starting 2 inches below right shoulder. Left arm, warm to touch raised red rash measuring 3in L x 4W, tenderness present. Vaccines were administered on 10/16/2023, mother state during time at home only zytec was administered. Pt was seen in clinic on 10/19/23 for evaluation of reaction by Dr., and second Dr. assessed pt. Treatment: Ibuprofen and Tylenol alternating for discomfort, cool compress. Instructed to continue to monitor adverse reaction, contact office with any changes.

Patient received the vaccine on 8/8/23. Rash/hives developed around 8pm 8/8 on bilateral legs, torso and arm. No viral symptoms. No fever. Rash is itchy. No new foods. No new skin products.

Age of patient was looked at wrong in (charting system), and gave what was appropriate for a 13 y/o (age thought patient was) instead of a 7 year old. HPV vaccine given on 9/20/23 at approximately 1710 to right deltoid. Parent of child was called the morning of 9/21/23 (once had interpreter available) and mom states child is doing well and has had no reactions.

Patient given Menquafi at 11 year and again at 12 year with no reported adverse event; Initial information received on 12-Sep-2023 regarding an unsolicited valid non-serious case received from a other health professional. This case involves a 12 years old female patient who was given Meningococcal A-C-Y-W135 (T CONJ) Vaccine [Menquadfi] at 11 year and again at 12 year with no reported adverse event. The patient's past vaccination(s) included MENQUADFI on 13-SEP-2022 with At 11 years of age. The patient's past medical history, medical treatment(s) and family history were not provided. On 08-Sep-2023, the patient received a dose of suspect Meningococcal A-C-Y-W135 (T CONJ) Vaccine Solution for injection at 11 years and again at 12 years (lot U7673AB, with unknown strength, dose, expiry date) via unknown route in unknown administration site as prophylactic vaccination. On 13-Sep-2022 and 08-Sep-2023, the patient received a dose of suspect Meningococcal A-C-Y-W135 (T CONJ) Vaccine at 11 years and again at 12 years with no reported adverse event (inappropriate schedule of product administration) (unknown latency). This case involves a 11-years-old female patient who was administered dose of meningococcal A-C-Y-W135 (T conj) vaccine [Menquadfi] after the second excursion (the temperature reached 0.5 c for 32 minutes) and after the first 3 excursion, with no reported adverse event while receiving vaccine. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. On 29-Aug-2023, the patient received 0.5 mL dose of suspect meningococcal A-C-Y-W135 (T conj) vaccine, solution for injection (batch number: U7673AB; expiry date: 29-Jun-2025) via intramuscular route in the left deltoid for Immunization after the second excursion the temperature reach 0.5 c for 32 minutes and after the first 3 excursion, with no reported adverse event (poor quality product administered) (same day latency).
