12 year Patient received Td when he should have received Tdap vaccine. Physician was consulted. The Td was appropriate for the age of the Patient, but the Tdap was required. Patient did receive the Tdap vaccine, as directed per Physician. There were no Adverse outcomes from the vaccines administered, other than the Patient received the wrong vaccine. As soon as the error was discovered, the Parents, the Nursing supervisor and the Physician on call was notified.
