Symptomtext

routine flu shot was administered today, pt was advised to premedicate w/ Benadryl (of which she took 75mg ~1hr prior to flu vaccine administration). 20min after flu shot she began feeling tingling and numbness in soft palate, tongue and lips; when sx progressed I administered patient self supplied epi-pen in left thigh, w/in 2min sx lessened. after about 20min into epi she started feeling paresthesia in mouth again, so I administer solumedrol 125mg IM into right deltoid. sx remained but did not progress, so no further epi was given. vitals remained stable and no further sx developed. pt was observed for 1.5 hours and remained stable, so was released home w/ husband for close monitoring.

Symptomtext

During administration, the needle was inserted in the left deltoid. When the syringe plunger was pressed, the needle almost immediately retracted (Vanish Point). The majority of the vaccine leaked out of the syringe and remained in the syringe. Little to none was actually injected into the arm. A full dose was given in the same day in the right arm.

Patient requested seasonal flu and shingrix vaccines. When documenting the Shingrix vaccine, noted patient had received 2 doses already and therefore the Vaccine given on 9/16/22 was not required resulting in an extra dose of the shingrix. She has received Shingrix on 2/11/22 , 4/29/22 and 9/16/22. Pt informed and provider APRN informed. Pt denies side effects as of 9/16/22.
