Symptomtext

Shot given just before 12pm on the 19th of September. Woke up the morning of 21st and noticed rashes on back of right thigh, as well as back and chest. She seemed to be acting totally normal and I read that sometimes vaccines can cause eczema to flare so I thought that was the case and since she acted fine I let her go to school. Complained of her butt itching when I dropped her off to school at 12:45 on the 21st. Was called to pick her up from school around 2pm because the rashes/hives were spreading. Upon getting there and seeing her she had rashes/hives on her arms now and a little on her face. Took her home and she went to my wife?s work later that day around 5pm and at that time the rashes/hives on her face had gotten worse. Doctor prescribed Zyrtec to control the issue. Symptoms cleared very quickly. Was told to continue Zyrtec for 4 days. The rashes/hives did come back the next day and cleared after another dose of Zyrtec. Same thing again the next day but less severe. As of today (5th day - no Zyrtec) it seems to be done with.

She saw the doctor on the 21st at 5pm as mentioned, and saw an allergist the next morning on the 22nd. Allergist suggested it may have just been a one time ?viral? rash/hives.

Given 4 yr shots - quadracil and proquad Later that day, mom noticed pt would have random mouth opening movement, almost like a sigh (Mom did notice him doing it in the few weeks prior to the shots, but now doing it much more frequent). Last night, mom call me while on call and noted that pt would have brief twitch of hand or lower leg did notice it during the day, but when sleeping did occur every 30 sec at one point. Mom had given benadryl as pt had some swelling of arm near injection site and later thought he was doing better. Discussed going to EC if increased frequency / sx. This morning at 5 am, mom noted twtich every few minutes in sleep. Discussed waking pt up and if stops and acting normal, can eval in office - if persistent go to EC. As pt seemed fine, mom came to office; noted one mouth opening in car. Will also have occasional very quick twitch. Pt is generally restless and moves a lot, though mom feels this is more than his usual

Symptomtext

Redness, swelling, hot to the touch and hard at the injection site the evening of the injections. Complained that it was painful to walk on his left leg as well.

Symptomtext

At 9 p.m., pt. started screaming and crying sporadically throughout the night saying his head hurt and leg hurt. He refused pain medication. His legs twitched throughout the night. In the morning, he cried his leg was sore and was limping. According to the handout received at the Health Department, these reactions can be a normal response to this vaccine. I took him to my mother who watched him the following day (8/3/23) while I worked. When I picked him up at 4:15 p.m., his left upper leg was very hard and very swollen and had red patches all over. The vaccine site was hard. His skin was hot to the touch. He continued to limp. He also continuously cleared his throat. I took him (and his older brother) home and when we arrived, my neighbor was outside; she is a nurse. I showed it to her and she said we should go to the ER. I called the pediatrician's office who also advised he should be taken to the ER. We arrived at the ER about 5:12 p.m. He was given oral steroids and I was told he was having a reaction to the vaccine and should never receive it again. We were released that night and he had a follow up appointment with a pediatrician the following day who prescribed a different dosage of steroid. He took the steroids for 5 days as prescribed. He continues to clear his throat constantly, has not slept through the night since receiving the vaccine, cries much of the night, has wet the bed every night since receiving the vaccine, and still complains of sporadic headaches. The injection site is still hard.

Patient came in for a 5 year well child visit. Patient needed MMRV and DTap for school. Patient was given Tdap (adacel) and Dtap (Quadracel) at 11:15 am 12/4/23. Documentation has been done and trying to notify parents.

patient came in for a well child visit on 12/21/2023. Was supposed to receive quadracel and Proquad. Patient received quadracel and pentacel instead. Called and reported error to the state and was advised patient may experience an exaggerated reaction (Increased tenderness, swelling, redness), adverse event brought to the attention of the covering provider. Per the state and Dr. parent was advised no harm in receiving extra dose of vaccine, just may see an increased risk for exaggerated reaction (above reaction discussed in detail above) Parents verbalized good understanding and will call with any additional questions or concerns. Patient is scheduled to come back for the proquad in December.
