Dysstasia Hyperhidrosis Syncope Vision blurred

Symptomtext

Client and family were exiting the facility when the client became unsteady on his feet and had an episode of syncope. Family helped client to the ground. Client had received 3 vaccines and a TB skin test. We assessed client and father helped client to an upright position and placed him in a chair. Client was diaphoretic. We gave him 2 bottles of juice. Vitals signs were taken at 2:19pm: BP75/30, HR 65. (per machine). BP rechecked manually at 2:25pm and was 110/56. Client did c/o blurry vision at the time he had the syncope episode. Family refused ambulance service. Client recovered and went home with family. Family given instructions to take BP every hour and drink lots of water by nurse practitioner and if in doubt take client to urgent care or ER for follow up.

Symptomtext

Patient brought vaccine record from home country. I entered vaccine record into state system. After entering and reviewing shot record, it appeared that patient was behind on DTAP and needed a Tdap shot. Tdap was then administered. After review of shot record later on 9/26/23, it was found that pentavalent from the shot record was not entered, thus patient did not need Tdap at that time. Mother of patient was contact on 9/27/2023 and was informed of this. Per mother of patient, patient did not have any adverse effects and reports patient only having a sore arm.

Would also like to note that she received a TB Skin Test of 0.1 ML of Tubersol as well on the left forearm. Patient started losing color in the face and closing eyes a lot. Patient did not end up fainting due to nurse interventions of juice and crackers, however, patient vomited after drinking some of the juice. Patient laid back in a chair recumbent and ate more crackers and drank more juice and color returned and child was totally fine. So, fainting did not end up occurring but she did vomit.
