Symptomtext

The patient scheduled an appointment for the meningococcal vaccine and requested the tdap prior to arriving at the pharmacy. Mom stated that the patient would climb under the table and use to be very afraid of vaccines which I believe made him nervous. The patient reported that he had eggs a 1 and 1/2 prior to his vaccination. After the vaccination the patient stood up from a seated position and reported that he felt like he was dizzy. The patient was asked to sit in a chair with his head between his legs for a few minutes. The patient was provided cookies and water and checked on multiple times during his visit. The patient felt well at 1:30 and left with his mother to grab a bite to eat. I reviewed the package insert to verify that both vaccines may be administered together.

Woke up with mild headache the morning of 9/8/23. She began with elevated HR up to 170 that day. Had mild chest tightness and lightheadedness also. Had a temp up to 99.7. Had back pain also. Continued with back pain, elevated HR and chest tightness on 9/9/23.

Symptomtext

patient received a duplicate dose of MENQUADFI, Boostrix (Tdap), and HPVwith no reported adverse event; Initial information received on 10-Oct-2023 regarding an unsolicited valid non-serious case received via other health professional. This case involves a 12-year-old male patient received a duplicate dose of meningococcal A-C-Y-W135 (T CONJ) vaccine [Menquadfi], boostrix (tdap), and hpvwith no reported adverse event. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included diphtheria vaccine toxoid, pertussis vaccine acellular, tetanus vaccine toxoid (TDAP) for Prophylactic vaccination; and HPV vaccine for Prophylactic vaccination. On 12-Sep-2023, the patient received a dose 1 of 0.5 ml dose of suspect Meningococcal A-C-Y-W135 (T CONJ) vaccine, Solution for injection (strength: unknown) (lot: U7903AA, expiry date: 31-Dec-2025) via intramuscular route in the right deltoid for immunization. On 06-Oct-2023 he also received a dose 2 of 0.5 ml dose of the same vaccine (lot: U7903AA) via intramuscular route in the right deltoid for immunization and patient received a duplicate dose of menquadfi, boostrix (tdap), and hpvwith no reported adverse event (extra dose administered) (latency: 24 days) following the administration of meningococcal A-C-Y-W135 (T CONJ) vaccine. It was unknown if any lab data/Test results available. Action taken with meningococcal vaccine (Menquadfi) was not applicable. Outcome: Unknown. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.

During system balance patients name appeared twice on the borrowed log one on 9/12/23 and once on 10/6/23. Pt. 1/12/11 received Tdap, Men A and HPV on 9/12/23 during office visit with Dr. Mom instructed to schedule a nurse visit for influenza vaccine since we had just ran out of the vaccine in office to give to our patients. Mom scheduled nurse visit on 10/6/23 for immunizations. Vaccines (Tdap, Men A, HPV) for this day were documented on the wrong patient on 9/12/23 by accident. During the nurse visit on 10/6/23 the systems didn't reflect previous immunizations given on 9/12/23. Patient was given Tdap, Men A, HPV and Influenza vaccine during nurse visit consent and Screening questions answered. Mom filled out the screening checklist for contraindications to vaccines for children and teens marking no to all questions even the question asking has the child received vaccinations in the past 4 weeks. Staff administered all vaccines that were due at this point using EMR information.
