PT passed out in chair about 2 min after vaccine admin. states she saw spots prior to passing out. was in and out within about 5 seconds. mom and np caught pt in chair, she did not fall or hit her head. Came back and said she was feeling better. Np put cool washcloth on head. mom states she gets nervous and weak prior to receiving vaccines

Symptomtext

PT received the FLU shot and RSV vaccine yesterday and woke up in the middle of the night with difficulty breathing. She does having a history COPD/Asthma and does use a CPAP machine at night. She said she never felt this way before even with her diagnosis. She wonders if it could be a reaction from the RSV vaccine. Followed up with pt 6 days later and Pt states she is feeling a lot better.. she saw PCP and got prescribed antibiotics and steroids for her copd exacerbation.

Pt was given TDAP Vaccine in left arm, he then received Covid Pfizer Vaccine ( Comirnaty) in right upper arm, we proceeded to administer his influenza vaccine (fluzone ) in the right upper arm, as the provider finished administering the last vaccine - which was the influenza vaccine, the patient stated, "Uh-Oh, my fingers are getting numb and I am getting light headed" The provider proceeded to have him lay down on the exam table. the patient became very lethargic, he never lost consciousness. He apologized for getting lightheaded and states he donates blood regularly and has become faint and lightheaded in the past when donating blood. We had him drink some water, we asked him if he had eaten - he ate about 2 hours before coming to get his vaccines. We watched the patient for approximately 20 minutes at 7:50 pm , we raised the head of the exam table , at 7:53 pm , he was walking around in the exam room and states he feels fine. Patient was escorted to his vehicle, gait steady, alert and oriented.

Symptomtext

After about 24 hours I felt effects from receiving my COVID booster and annual flu shot. Symptoms presented on the right side of my head, mostly in the hairline. The skin was painful without even touching my skin. The pain periodically radiated into my right ear canal. On the fifth day of symptoms unabated and not improving, I sought care from local walk-in facility. The PA quickly diagnosed my condition as shingles. He stated that my shingles became active after my body had experienced stress, including my immune response to receiving two different vaccines. At this writing I am my first course of anti-viral caplets three pills a day for seven days.

I am having torticollis-like spasms in the right side of my neck in the C2-C4 areas. It wakes me up. It comes and goes. It has a sharp deep pain that hits for a second and comes and goes in 10-12 second increments or 10-12 minute increments.

Symptomtext

Patient reported hives-like reaction only covering the upper part of the arm in the vaccinated arm. She said it appeared the same as the hives she had in the past with a medication she's allergic to. She's otherwise fine & okay to work & carry on as normal.

Symptomtext

Patient called the pharmacy about 45 minutes after vaccination and reported that they were experiencing dizziness. They were advised to rest under the supervision of the people who they were with at the time and to seek medical help if the symptoms worsened. When I followed up a couple hours later they reported they had ended up going to the ER via ambulance. When asked about treatment they said the ER was taking forever and they ended up walking out because they started to feel much better.

Symptomtext

Immunizing technician was administering the flu vaccine to patient. The technician prepared to give the vaccine and administered the stick. However, after the stick the plunger would not move and would not allow administration of the vaccine. This was most likely a result of the stick occurring to high on the arm based on the placement of band aid and where the actual stick occurred. When this happened, the technician notified the pharmacist through the window. The pharmacist immediately came out and observed the situation. The pharmacist observed that the placement was too high and removed the needle. In addition, it is believed that no amount was administered during the process.
