Symptomtext

I was contact from the school secretary that the child was feeling short of breath after receiving the flu vaccine. They were able to assist her to the ground, laid her down and applied a cool wash cloth to her head. I instructed her to check the child's oxygen level and continue to monitor and let me know what the results were. Her sats were fine, she continued not to feel good, so they called the child's mother and she went home for the day. No other information or symptoms were reported thereafter. Prior to the injection the child was anxious, crying and did not want to receive the injection as she was scared.

Mom reports got their shots, they were fine, fussy, gave motrin 6p, he woke up fussy just bf MN, mom gave tyl, then mom heard something on monitor early AM 5:26am, looked convulsing, bilat shoulder twitching/eyes up and back, arms up/shaking, they got him out into living room, called 911, around 5min, then clinched jaw and eyes still kinda up, couple min maybe, dad thought 10min, but less than that, lots of spit in mouth. Plan is to monitor for further symptoms and then separate the MMR and Varicella vaccines at age 4.

48 hrs after receiving both flu and COVID19 booster, patient developed vomiting. She vomited several times per day and did not have other viral symptoms such as diarrhea. Also low appetite and fatigue. Nobody else in the family had similar symptoms. today is the 6th day of vomiting.

Mother states that a day after vaccination, the pt was running a fever. 4 days after the vaccination occurred, mother took pt into PCP with c/o a fever off and on for past 3 days. The mother states that she has given the pt acetaminophen for the fever. The mother states that the pt was dx with an ear infection and prescribed amoxicillin. The mother states that there is a lump on the pt's left leg where vaccination occurred, and skin is red and splotchy all over the pt's body. The pt's PCP told the mother that "it was probably a viral infection".

NOTE FROM THE ER The right arm does appear to have a post injection inflammatory reaction. You may use ice pack over the area, Tylenol and ibuprofen for pain relief. Today, we marked the red area with a skin pen. Please watch the area to ensure the redness is decreasing and does not expand be on the area marked off. If the redness expands beyond the marker, please start the antibiotics. Otherwise, anticipate this will resolve spontaneously without the need for antibiotic. MOC TOOK CHILD TO ED 12/14/2023. CHILD HAD NO SX. PATIENT ASKED FOR A BAND-AID WHEN HE GOT UP 12/14/2023. (PATIENT SAYS SWELLING STARTED 12/13/2023 AT BEDTIME) WHEN MOM LOOKED AT THE ARM ON THAT MORNING SHE COULD SEE THE SWELLING FROM RIGHT SHOULDER TO ELBOW.

On November 8, 2023 when child woke up left thigh had a large softball sized hard knot that was raised, hot to touch and painful to touch. The site was also itchy. Mother presented child to ER. Treatment prescribed was hydrocortizone cream and cold compress.

Symptomtext

Patient was vaccinated in clinic and waited with mother for 15 minutes, no adverse reactions were noted. Patient's mother came back to the clinic one week later for a separate appointment and stated that patient had redness, swelling and a bump at the injection site on patient's left arm the day following the injection. Patient did not have a fever. Mother gave patient heat pack, ice pack and Tylenol on Thursday. Patient's mother gave Benadryl on Friday (10/13/23.) On 10/18/23, patient still had a bump at the injection site but no redness or swelling.

Pt reports localized swelling and warmth to the left upper arm that occurred approximately 12 hours after administration. Recommended cold compress/ice pack to the area as well as OTC children's ibuprofen or Tylenol. Appt for eval scheduled at the pt's PCP clinic the day after vaccine administration.

Symptomtext

Patient received flu and covid shot and seemed to be fine. Approximately 30 minutes later, they were present during immunizations administered to 12 y.o. sister. She was standing behind mother against the wall in the clinic. She fell backwards hitting head on wall and fell to floor with limited responsiveness and limp. Eyes open but not following commands, limp extremities. Lying on floor with head supported.
