Patient reports experiencing extreme leg weakness to the point he was unable to stand and needed to call an ambulance. He was admitted to hospital were Urine, CBC and Other (pt could not recall other test names) tests were ran, but no significant findings were found. After spending 1 night in the hospital some of the strength has returned, but he still reports more weakness than usual. According to his wife he has a history of weakness in his legs/ difficulty walking , but not to the extent he is currently having.

Client came in for flu vaccine. Discussed with client whether she wanted regular flu or high dose flu vaccine. Client stated wanted high dose vaccine. When inputting into system, most recent flu vaccine in system was given on 10/5/22. Administered high dose flu vaccine to client. Approximately 20-30 minutes later client calls asking if she received the flu vaccine or COVID vaccine, told client she received the high dose flu vaccine. Client then states that she received the flu vaccine already a few weeks ago and intended to get the COVID vaccine, not the flu vaccine. Told client that I could call her PCP, on her behalf and she agreed to this.

This case involves a 72 years old female patient who received INFLUENZA QUADRIVAL A-B HIGH DOSE HV VACCINE [FLUZONE HIGH-DOSE QUADRIVALENT] and the pharmacist was not sure how much went in the patient/1 dose leaked with no reported adverse event. The patient's past medical treatment(s), vaccination(s) and family history were not provided. It was unknown if the patient had any medical history, concomitant disease or risk factor. On 28-Sep-2023, the patient received suspect INFLUENZA QUADRIVAL A-B HIGH DOSE HV VACCINE and the pharmacist was not sure how much went in the patient/1 dose leaked with no reported adverse event (incorrect dose administered) and (underdose) (latency: same day) (formulation unknown) (lot U8130AA and expiry date: 06-Jan-2024) (Frequency = once) (Strength = standard) via intramuscular route in the left deltoid for immunization. It was reported that "They want to know if there any precedent for giving another dose since they are not sure how much went in the patient." It was unknown if there were lab data/results available. It was unknown if the patient experienced any additional symptoms/events. Action taken: Not applicable. At time of reporting, the outcome was Unknown for both the events. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.
