ON 10/28/23, PATIENT CALLED THE PHARMACY AND COMPLAINED ABOUT SLURRED SPEECH, HEADACHE, AND TROUBLE WALKING. PATIENT SAID SYMPTOMS STARTED ON 10/27/23. PATIENT WAS ADVISED TO SEEK MEDICAL TREATMENT IMMEDIATELY. PATIENT WENT TO LOCAL HOSPITAL, WAS DIAGNOSED AS HAVING HAD A MILD STROKE, AND WAS ADMITTED.

On 10.31.23, Pt returned to dr office for routine follow up. At that time she stated left shoulder pain since flu shot. trouble elevating shoulder and rotating internally. Dx shoulder bursisits since flu shot. ortho referral for cortisone injection. can not take NSAID's due to kidney disease. On 11.01.23, seen for left shoulder pain, limited range of motion, stiffness after flu shot. Pain reaching overhead, interfers with ADL, no hx of similar problem before flu shot. hx of right rotator cuff tear with repair. Exam documented, Xrays show mildly high riding humerus, mild degenerative changes of AC joint. positive impingement test and restrictions overhead. Inject rosuvastatin. Injection may have pecked the bone or entered the subacromials space, bursa. Cortisone injection givenin subacromial space. Physical therapy ordered, tylenol 3 times day, ice/heat, elevation prn. 12.12.2023, ortho visit for left shoulder aching, sharp pain with limited ROM. Pain did not improve with cortisone injection and PT, only temporary relief. MRI ordered to rule out supraspinatus tear. MRI scheduled for 12/21/23.

This patient requested a flu shot on 12/27/23, so we went to her room in the long term care facility to administer the shot on 12/28/23. I logged on to immPRINT on 12/29/23 to check that our submission went through. When checking her profile, I noticed she had already received the flu shot by her PCP in September. I am submitting this report as she received two flu shots this season.

Symptomtext

As I depressed the plunger on the Fluzone HD quad while it was in the patient's left deltoid, the majority of the vaccine leaked out of the side of the Luer Lock down the patient's arm and onto the floor. The patient was advised that they should receive a repeat dose since they did not receive the full vaccine. It was indicated that they would return in a few days time to get a fresh Fluzone HD quad administered. The patient did not actually suffer any adverse effects from the vaccination itself.

Symptomtext

Patient reported swelling of arm beginning approx. six hours after vaccination. Patient stated that it was hard and warm over night. Patient did take benadryl. Reported that swelling was going down this morning and resolving at 9am (approx. 20 hours after vaccination). Patient is related to healthcare professional and was told vaccine was administered to low on the arm.

a day after patient received the fluzone high-dose quad vaccine she came into the pharmacy and said she had requested the covid vaccine instead of the flu vaccine and that she had already received the flu vaccine at a hospital several weeks before. she said she had no ill effects from the vaccine she had received the previous day.
