Symptomtext

Received the vaccinations at 16 month appointment. Was on target for all well check areas. By 18 month appointment-speech had become less and now to the point of nonverbal at 23 months. Sleep patterns are terrible. Constipation frequently. Eating habits changed. Eye gaze. High and lows of emotions. Slow reaction to pain situations. Non affectionate. Doesn't respond to verbal cues at all. Does not interact with other kids his age. 5 days after vaccines on 08/04/2020 he was taken to Urgent Care for high fever, no eating, no wet diapers and being lethargic. On 08/26/2020 was taken back to pediatrician for fever, no eating, no sleeping. Was told it would pass. On 08/28/2020 was taken to pediatrician for for same issues. Did a full blood screening, 2 catheters attempts, still high fever, lethargic, not eating. Fever finally subsided on 09/01/2020. Pt. still received the polio vaccine and hep b vaccine on 09/04/2020. Pt. still received the hep a vaccine on 10/13/2020.

2 expired PENTACEL vaccines used,no AE; Initial information regarding an unsolicited valid non-serious case was received from other health care professional via Agency (Reference number-00536990) and transmitted to Sanofi on 26-Mar-2021. This case involves a 4 month old female patient who was administered an expired dose of DIPHTHERIA/TETANUS/5 HYBRID AC PERTUSSIS/IPV(MRC5)/HIB(PRP/T) VACCINE (lot UJ146AAA, expiry date: 14-Feb-2021) via unknown route in unknown administration site for prophylactic vaccination on unknown date (expired product administered). Medical history, medical treatment(s), vaccination(s), concomitant medications and family history were not provided. It was a case of an actual medication error due to expired vaccine used (latency same day). Nurse states that 2 expired PENTACEL vaccines were administered. Patient received an expired Pentacel. At the time of reporting, patient had no adverse event. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.

This case involves a male patient (child) who received expired dose of DIPHTHERIA/TETANUS/5 HYBRID AC PERTUSSIS/IPV(MRC5)/HIB(PRP/T) VACCINE [PENTACEL]. Medical history, medical treatment(s), vaccination(s), concomitant medication and family history were not provided. On 24th (month unspecified), the patient received a dose of suspect DIPHTHERIA/TETANUS/5 HYBRID AC PERTUSSIS/IPV(MRC5)/HIB(PRP/T) VACCINE (lot UJ146AAA, expiry date- 14-Feb-2021) via unknown route in unknown administration site for prophylactic vaccination. Ipol and the other vaccine, Pentacel and both have polio in the vaccine with no AE; Initial information received on 07-Jan-2021 regarding an unsolicited valid non-serious case received from other health professional. This case involves a 2 months old male patient who received a 0.5 ml dose of vaccine QUADRIVALENT INFLUENZA VACCINE [FLUZONE] (lot: UJ146AAA, expiry date: 14-Feb-2021) and POLIO VACCINE INACT 3V (VERO) (IPOL POLIO VACCINE) (lot number and expiry date unknown), both via unknown route at unknown administration site, on an unknown date (wrong product administered) as prophylactic vaccination. The patient's medical history, medical treatments, vaccinations, and family history were not provided. Concomitant vaccines included HEPATITIS B VACCINE and PNEUMOCOCCAL VACCINE 13V. It was an actual medication error due to ipol and the other vaccine, pentacel and both have polio in the vaccine (Extra dose administered). At the time of reporting, the patient had no adverse event. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices.
