Symptomtext

Per emergency room note, mom reported that patient started jerking both arms around 20:50pm and did not seem like herself. She was brought in to the ER via EMS and found to be in status epillepticus. This occured about 10 hours after getting 4 months old vaccines.

Symptomtext

Vaccine Nurse inadvertantly diluted ActHIB with sterile water diluent instead of included normal saline diluent. Pt's father was immediately notified of the event and the need to repeat vaccine. Father provided consent, vaccine was readministered after appropriate reconstitution with ActHIB diluent. Patient tolerated well.
