Pt's mother reports that pt was brought to Hospital ED at 3AM and 12PM 8/11/21 - Pt w/ reaction R arm one day after vaccine administration - R arm red/purple swollen, firm, hot to touch, pt scratching at arm and crying out/pulling away when arm touched - Pt w/ fever and poor appetite - Pt brought to ED, given allergy medication, motrin, and abox Pt now with smaller area of redness and swelling, reduced warmth, mild congestion and nasal drainage Mother continuing to monitor pt, declined f/u appt, will cb tomorrow if continues or gets worse.
