Symptomtext

Child received DTAP & HIB at clinic thursday 3/4/21 at around 4pm. Child developed fever Friday evening 3/5 and on Saturday evening 3/6 developed a seizure to which parents called 911. Seizure described as b/l arms flex/ eye deviation. Seizure stopped by time 911 arrived. Child taken to hospital by EMS. Father states that during the course the child had a Tmax of 40C/ 104F. rapid covid test negative. No other sick contacts at home. Older brother in school- elementary in person. Observed in the ED x 4-6 hrs and cleared for discharge as simple febrile seizure. Unclear if had gotten tylenol with fever. Child returned to baseline, looks and acting well, child does have decreased appetite. Parents have been giving Tylenol round the clock. this morning Mon 3/8, temp was 100.4/38C , retemped at 11am - and had a temp of 37.1C afebrile but mother still gave tylenol. at that time. In the clinic, at approximately 4 1/2 hrs after tylenol he was afebrile at 99.1, has scheduled f/u Wed March 10th.

Rash Urticaria

Symptomtext

Mild facial hives/rash noted roughly 10-15 minutes after getting vaccination. Patient was referred to local ER. Rash/hives self resolved per family several hours later. Pt did not get any treatment for rash since family left ER prior to being seen.

Medication error Product preparation error

Symptomtext

ACTHIB reconstituted with 0.9%NaCl; Initial information received on 18-Mar-2022 regarding an unsolicited valid non-serious case received from a other health care professional via call center. This case involves a 12 months old male patient who reported acthib reconstituted with 0.9%nacl while receiving vaccine Hib (Prp/T) Vaccine [Act-Hib]. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included (Hepatitis Vaccines) for Prophylactic vaccination; Varicella Zoster Vaccine (Varicella Vaccine) for Prophylactic vaccination; and Pneumococcal Vaccine Polysacch 23V (Pneumovax) for Prophylactic vaccination. On 17-Mar-2022, the patient received a dose of suspect Hib (Prp/T) Vaccine at dose of 0.5ml (formulation and strength: unknown)(lot number: UJ524AA and expiry date: unknown) via intramuscular route in unknown administration site for prophylactic vaccination. On 17-Mar-2022, the patient reported a Act-hib reconstituted with 0.9%nacl (product preparation error) following the administration of Hib (Prp/T) Vaccine. Action taken: not applicable. At time of reporting, the outcome was Unknown for the event product preparation error. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.
