Symptomtext

The Pentacel Diluent (DTaP./IPV) was mixed with the powder Rotavirus and given IM. The Nurse realized her mistake after she returned to her work area. The parent, Doctor and Nursing Team Leader was notified of the error. The Nursing supervisor contacted and Sanofi Pasteur and learned that the patient could have a slight local reaction. We instructed the Mother to let us know if child has any issues within the next 48 to 72 hours and if showing signs of an allergic reaction to go to the ER.

This case involves a 6 months old male patient who experienced part of the vaccine came out of the hub area of the syringe and ran down on the table with no reported adverse event while receiving vaccine DIPHTHERIA/TETANUS/5 HYBRID AC PERTUSSIS/IPV(MRC5)/HIB(PRP/T) VACCINE [PENTACEL]. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included HIB VACCINE CONJ (TET TOX) (ACTHIB) for Prophylactic vaccination; HEPATITIS B VACCINE (HEPATITIS B VACCINE) for Prophylactic vaccination; ROTAVIRUS VACCINE (ROTAVIRUS VACCINE) for Prophylactic vaccination; and PNEUMOCOCCAL VACCINE CONJ (PNEUMOCOCCAL CONJUGATE VACCINE) for Prophylactic vaccination. On 14-Jan-2022, the patient received a dose of suspect DIPHTHERIA/TETANUS/5 HYBRID AC PERTUSSIS/IPV(MRC5)/HIB(PRP/T) VACCINE (lot UJ597AAA and expiry date was 05-Dec-2022) via intramuscular route in unknown administration site for prophylactic vaccination. On an unknown date the patient developed a non-serious part of the vaccine came out of the hub area of the syringe and ran down on the table with o reproted adverse event (incorrect dose administered) (unknown latency) following the administration of DIPHTHERIA/TETANUS/5 HYBRID AC PERTUSSIS/IPV(MRC5)/HIB(PRP/T) VACCINE. Action taken with DIPHTHERIA, TETANUS, AC PERTUSSIS, IPV AND HIB VACCINE (PENTACEL) was not applicable.
