Rash Rash erythematous Wheezing

Symptomtext

Within 1 minute of immunization he had onset of erythematous rash on L thigh. Over the next x2 days rash spread to head, neck, arms, & trunk. He had audible wheezing. No respiratory distress, angioedema, anaphylaxis. He is now x6 days from immunization and is completely resolved. Did not have similar reaction to previous HepB or Rotavirus.

Symptomtext

MMRV was given at this appt instead of varicella alone. Also MMR was given as well. Therefore the child received two doses of mmr today at the appointment which was a mistake. the MA reported the error, and the mother was made aware.
