Fontanelle bulging Infant irritability Pyrexia Ultrasound head abnormal

Symptomtext

fever and markedly buldging fontanelle starting twenty four hours after administration. Fever to 102 that lasted three days. Took three days for fontanelle to become non- tense, four days to not be buldging and five days for the fontanelle to become sunken again. Infant fussy but feeding. No vomiting or neurologic sympotms . Healthy infant with prior progression of milestones Interestingly similar reaction after two month vaccines - pentacel, PCV13 and rotavirus. First time buldgin fontanelle with NO fever that lasted a couple of days. Dramatic presntation so sent ot neurosurgery and US that showed benign minimal prominence of the subarachnoid spaces

Rash Urticaria

Symptomtext

Patient woke up from afternoon nap with rash/hives. No fever or difficulty breathing. Parents sought care at urgent care. Seen by APRN student & licensed APRN. Diagnosis-hives. Prescribed Benadryl 1.25ML every 8 hours for 2 days AND Zyrtec 2.5ML daily for 1-2 weeks. No follow-up visit scheduled. Message left for parent to return call to Health Department; call never returned.

The medical assistant did not attention that the Hib vaccine was not available in the refrigerator and mistakenly applied pentacel confusing the vaccine at the time of documenting in the chart they Patient check that it was the wrong vaccine and reported to the Provider
