Received Influenza Vaccine 9/16/21. Two days later presented to our ER with dysphonia and dysphagia. MRI/MRA showed no sign of acute stroke. Consult with Dr. suggested possible variant of Guillan Barre Syndrome. Transferred to Facility. He was intubated initially to protect his airway. Subsequently extubated and died. Diagnosis of Myasthenia Gravis was also considered and test has subsequently come back positive for this antibody.

patient started to have chills, burning in hands and forearms, as well as a feeling like she could pass out after returning home from vaccination. She had some chills and stated her blood pressure was 80/40 when she checked it. she did not call the pharmacy/vaccinator nor her physician. she took a nap and woke up with a pretty bad headache. She had taken some tylenol prior to the nap. She called us the following day to report that she had issues.

He got his vaccine, as soon as he got it he got a stiff neck. The next day he got a headache, an then really sick to his stomach and took Pepto Bismol and went to get out of his chair on Monday following the vaccine and he fell to the ground and had excruciating pain in his left leg and back, which he has never had this problem before. He had to use a walker as his leg gave out on him, which the lower back and leg pain has continued which is down to his feet and his feet are swollen. He also has pain in his jaw on the left side. Everything is on the left side. He then got a big huge blister on his elbow, which is starting to heal up. He went to the ER twice, he went to his PCP and his endocrinologist and received steroids, muscle relaxants which did not help. He cannot recline back as the pain is excruciating and has not slept in his bed since 9/13/21. The pain is starting to subside, just a little bit, it is now a 7/10 now, but was a 10/10, and he cannot do anything as he now has to walk with a walker, which he did not need a walker before the vaccine. He had a regular appointment with his PCP today who was shocked to see the blister on his arm. He saw an orthopedist and they have him scheduled for an MRI on 10/28/21 of his spine. His endocrinologist sent him for COVID testing and also for Lyme testing and they both came back negative. The doctors that he has seen cannot figure out what or why he is having these symptoms. He feels that this is reaction to the vaccine. He did in the past had elbow and shoulder surgery on the left side 34 years ago and never had a problem, and feels that it has attacked his weak spots from previous surgeries. He has a few friends that are now in wheelchairs from the same vaccine. He also has blisters on his little finger and his ring finger on his right hand, and the doctor said that it was deep and that he had never seen one that deep. The skin has been healing and was extremely thick and that it took half of his knuckle off of that hand, which is the only thing that has happened on the right side.

Symptomtext

A patient received a partial dose of FLUZONE QUADRIVALENT HIGH DOSE due to the vaccine leaking no AE; vaccine leaking out from between the needle and the connection tip on the syringe no AE; Initial information regarding an unsolicited valid non-serious case was received from a pharmacist via a consumer/non-health care professional via Medical Information (Reference number- 00829551) and transmitted to Sanofi on 26-Oct-2021 a specific country. This case involves a 66 year old female patient who had received a partial dose of INFLUENZA QUADRIVAL A-B HIGH DOSE HV VACCINE [FLUZONE HIGH-DOSE QUADRIVALENT] due to the vaccine leaking from between the needle and the connection tip on the syringe (underdose and syringe issue). The patient's medical history, medical treatment(s), vaccination(s) and family history were not provided. The patient had no concomitant vaccination. On 26-Oct-2021, the patient received a dose of suspect INFLUENZA QUADRIVAL A-B HIGH DOSE HV VACCINE (lot UJ709AA, expiry: 30-Jun-2022, formulation: prefilled syringe) via intramuscular route in the left arm for prophylactic vaccination. It was a case of an actual medication due to Underdose and Syringe leak (latency same day). It was reported "Pharmacist says that while a technician was administering a dose of FLUZONE high dose (HD) some of the medication leaked around the needle. Caller wants to know if they need to re-administer the medication to the patient. A Pharmacist reporting on behalf of a consumer getting a partial dose of FLUZONE QUADRIVALENT HIGH DOSE due to the vaccine leaking out from between the needle and the connection tip on the syringe. Pharmacist reported that the consumer may have received half of the dose." No other adverse event was reported. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.

Patient presented for flu vaccination at work site. Nurse reviewed front of form and read the patient's age as 66 instead of 60. Nurse did not review back of form which had additional check for patient's age. She offered the high dose to the patient which he accepted. Reviewed with nurse workflow to avoid future error like this. She stated understanding. F/U call made to patient on 9/13 to explain what happened. He understands and did not have any questions.
