Employee feels like injection was not given in the right location. Employee stated she had dysfunction and shoulder pain. Employee received another vaccine within 30 days of this incident occurring in the same arm. Employee reported incident 3 months later after making appointment with her PCP. She was given steroid injection on 12/20/21.

The patient waited the required 15 minutes. At that time the patient felt lightheaded and requested water. The patient and his mother reported that he did not have anything to eat that morning. The patient was given 1 granola bar and bottled water. Vitals: BP 100/65 P 66 T 97.1. The patient consumed the food and reported feeling better.

Swelling and redness at injection site. Warm to the touch. Low grade fever and achy. Seen in office today. Instructed to take scheduled ibuprofen and cool compress to site. If progresses/worsens stop ibuprofen and begin medrol dose pack.

Adverse event: Severe bilateral hip joint pain and inflammation that left me unable to walk and unable to conduct normal life functions without severe pain for approximately 38 hours. Treatment: Over the counter pain relief, heating pad, ice packs, laying flat on my back. Outcomes: At around 38 hours after the onset of the severe bilateral hip joint pain and inflammation that left me unable to walk and unable to conduct normal functions without severe pain the pain subsided.

Symptomtext

Vaccine administered 9:45am, Symptoms started a few minutes later. Feeling like something was inside her throat, coughing, clearing throat, metallic taste in mouth. Epi-Pen administered 9:50am. Symptoms better, not completely resolved. . 10am Prednisone 60mg and Benadryl 50mhg orally ordered by our MD and administered. Observed in nursing until 10:30am.
