Symptomtext

Patient's daughter called the pharmacy stating that the patient developed blood clots after the vaccine, which resulted in the patient being hospitalized on 10/12/21. Patient's daughter said her mother's doctor and nurse practitioner claimed that the flu and COVID vaccines should have been separated by 14 days, but we explained that the CDC recommendation at the time was that both could be given at the same time (one in each arm, or both in the same arm at least an inch apart). We are not sure what tests were done while the patient was hospitalized, but the patient's daughter insisted that a report be made to VAERS in response to the incident. Patient has since been discharged from the hospital and is now under home healthcare for clot treatment.

Per Patient: Severe headache with aches and pains all over my body! Uncontrollable violent shaking , freezing, arms and hands going to sleep , vomiting , feeling like I was going to pass out and extreme swollen lip when I woke up yesterday morning! This went on for 11 hours!

Symptomtext

Patient stated vaccine caused throbbing arm pain & 2.5 hours post-adm, he returned to pharmacy and I checked area, no indication of any vaccine error. Pt had stated to adm tech, that he received a flu shot at walgreens last year and they "gave it too high" so he had to go to his PCP to get a steroid shot. I advised patient to take tylenol or IBU, and ice the area and I would call him in the morning. When I called him this AM, he said he tried to go to urgent care and get a steroid shot, but they would not give it to him. Per patient, they said everything looked normal and he needed to continue OTC pain meds and ice. He said at noon today, he is feeling better and will continue to treat. All normal arm soreness post vaccine. advised pt to receive all future vaccines in alt arm.

HER HUSBAND SAYS SHE WOKE UP THE NEXT DAY WITH REDNESS AND SORENESS ON THE INNER ASPECT OF HER LEFT ARM (THE ARM SHE GOT THE SHOT IN). THE AREA WAS ABOUT 6 INCHES LONG. AS OF 9-30-21 SHE STILL HAS A RASH.

This vaccine was given and unfortunately we missed the patient's age and gave her a 65+ HD vaccine when she is only 60. We did not realize the mistake until we came back to the store and processed the vaccine.
