Streptococcal pneumonia with acute hypoxic respiratory failure -Urine antigen positive for strep pneumo, patient needed supplemental persistent oxygen to maintain saturations above 90%, patient initially treated with IV Rocephin, her oxygen requirement decreased with time and now she has not qualified for home oxygen. Will give a week of Augmentin post discharge. ED visit to hospital admission within 6 weeks of receiving the COVID vaccine.

Acute STEMI presentation - initial cath with no culprit lesion identified. with history of type 2 diabetes, permanent atrial fibrillation with intrinsically controlled heart rates, and long term anticoagulation with Xarelto, Obesity, hypertension , hyperlipidemia, DVT and right sided carotid endarterectomy admitted for further evaluation of chest pain progressively worsening since receiving COVID vaccine booster 2 days prior. ECG was consistent with anterior STEMI. Initial emergent LHC was felt to show TIMI 3 flow in all vessels and moderate diffuse disease - the films were reviewed by multiple interventionalists and the consensus was that no culprit vessel could be identified. ED and hospital admission was within 2 days of receiving the COVID vaccine.

Symptomtext

Patient fell and had swelling and a large abrasion to the forehead, extending into the scalp. ED visit; Patient given tramadol for pain with a small prescription for this. Hospital admission for history of progressive left vulvar edema tenderness and febrile sensation. Incision and drainage was performed with placement of a Word catheter. She was deemed to have achieved maximal benefit of inpatient therapy on the third postoperative day and discharged to home care on doxycycline. ED visit and hospital admission were separate and both within 6 weeks of receiving the COVID vaccine.

spoke with patient she is experiencing an itchy arm, redness around the site, and soreness. she did notice a little swelling and has a lump the size of a plum. per Dr apply a cold compress, monitor for a fever, take ibuprofen and Zyrtec for pain and itching.

Symptomtext

Customer came in for a flu vaccine, I used an Inject-Safe-Barrier Bandage. Customer left the vaccine area to shop for groceries but returned 10-15 minutes later and said the injection site had bled out of the bandage. I brought her into our immunization room and took off the Inject-Safe-Barrier Bandage and cleaned her injection site and placed a regular bandage over the injections site after the bleeding stopped. Patient stated she had just recently started a new blood thinner and had been rotating her arm while shopping to work out any soreness.

SHE STATES THAT SHE BECAME THIRSTY, HAD FEVER BLISTERS, HER CHEST BONE HURT AND HER CHEST WAS HEAVY. SHE HAD SPASMS IN HER BACK AND DOWN HER LEGS. SHE SAID SHE DROVE TO HER SONS AND THIS LASTED APPROXIMATELY 15 MINUTES. SHE SAID LATER SHE HAD FLU-LIKE SX THAT LASTED 11-12 HRS. I TOLD HER I WOULD REPORT AS THESE WERE RATHER UNUSUAL SX.
