Symptomtext

Pt was here for his 17yr physical exam today at 8:30am. When exam was completed, I administered the ordered vaccines: Administered Influenza and Menactra in Left deltoid, then administered Trumenba in Right deltoid. Immediately after giving the Trumenba, pt leaned back against the wall and was grabbing his right arm. I asked if he felt ok, he said yes. I asked if he has eaten anything yet this morning and he replied no, so I gave him a juicebox. I advised pt and his mother the concern for possible passing out due to receiving vaccines early in the morning on an empty stomach. Mom assured me he was ok and I left the room. Around 9:15am, I was alerted to patients status and went into the exam room. At that time, pt was sitting in a chair and was extremely pale. Immediately alerted provider and she came into the exam room. His eyes rolled back and seemed to have passed out, was unresponsive for about 5-10 seconds. Pt then opened his eyes and was responsive and vomited small amount onto the floor. He was weak and sweating. Applied icepack to back of his neck, then was given water and some pretzels. Continued to have pt sit and was monitored by RN, for about 10 more minutes. At that time, pt reprted he was feeling normal. Pt and mother felt they were comfortable to leave, they left the office around 9:31am. Advised mom to please notify office asap of any changes or symptoms today, also notify us if pt develops fever or chills tonight.

Crying Fall Syncope

Symptomtext

Pt fainted after shot while ambulating. She fell forward face down rolling onto her shoulder. Mom picked up patient from floor, placed on exam table on back. We removed sock and shoes, turned on fan, patient woke up within seconds crying. Juice and popsicle given vitals were HR 116 SPO2 99%. Observed for > than 30 minutes patient ambulated without difficulties. Spoke with mom 2 days later. States she spoke with pediatrician no need for office visit. States child has normal behavior no further reactions or evidence of injury. No further care was needed.

Symptomtext

Mom reports red size raise welt at Right Deltoid area, about the diameter of a soup can. Patient is scratching at it so mom doesn't know if it is causing pain or itch and patient has sensory issues due to autism so not able to really verbalize clearly what the discomfort at arm is. Not oozing at site. No other swelling reported at extremity at this time. Mom reports hot to the touch. Mom has only tried cold compresses. Patient is afebrile and no changes in behavior. Mom applying cold compresses, giving tylenol OTC and advised to mark margins so as to see if it enlarges. No swelling or other injury noted to extremity. Mom asked to call back or seek urgent care if no improvement or worsening of symptoms.

gentleman who received Fluzone High dose Quadravalent today but was advised that he also received the same vaccine only four months ago with no reported adverse event; Initial information received on 18-Feb-2022 regarding an unsolicited valid non-serious case received from a other health care professional. This case involves a 83 years old male patient who received fluzone high dose quadravalent today(18-Feb-2022) but was advised that he also received the same vaccine only four months ago with no reported adverse event while receiving vaccine Influenza Quadrival A-B High-Dose HV Vaccine[Fluzone High-Dose Quadrivalent]. The patient's past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications included Varicella zoster Vaccine RGE (CHO) (Shingrix) for Immunisation. On 18-Feb-2022, the patient received a 0.5ml dose of suspect Influenza Quadrival A-B High-Dose HV Vaccine(formulation: unknown, Strength: standard) (lot UJ775AB & expiry date:30-Jun-2022) via intramuscular route in the left deltoid for immunization.Therapy (1st therapy patient took in Oct-2021) (dose: unknown). On 18-FEB-2022, the patient who received fluzone high dose quadravalent today(18-Feb-2022) but was advised that he also received the same vaccine only four months ago with no reported adverse event (inappropriate schedule of product administration)(latency: unknown) following the administration of Influenza Quadrival A-B High-Dose HV Vaccine. Action taken: not applicable. At time of reporting, the outcome was Unknown for the event inappropriate schedule of product administration. This suspected adverse reaction report is submitted and classified as a medication error solely and exclusively to ensure the marketing authorization holder's compliance with the requirements set out in the Directive 2001/83/EC and Module VI of the Good Pharmacovigilance Practices. The classification as a medical error is in no way intended, nor should it be interpreted or construed as an allegation or claim made by the marketing authorization holder that any third party has contributed to or is to be held liable for the occurrence of this medication error.

Symptomtext

Patient reported right shoulder, pectoral muscle and arm pit swelling overnight with visible signs upon waking. Tender to the touch at point of swelling (not at injection site). Patient reported to the pharmacy Monday morning and said it was improved, still a little sore but swelling was down. No fever or other symptoms to report - not even sure it was from the shot but the timing coincided within 12 hours of administration and patient did not knowingly experience any other events that would have precipitated the reaction.
